I've been on TRT With Defy Over a Year - Still Can't Figure Out Appropriate Dosage

Some may feel more comfortable not making such a large jump from 140 mg/week - 100 mg/week.....................sure only dropping to 120 mg/week may not result in lowering levels enough but from the mental aspect some are nervous when having to decrease testosterone dose especially when they are so use to a said protocol.

That's exactly why I was hesitant to decreasing down to 100mg a week.

I see the point but I don’t think it is nothing to be afraid of... specially with such very high FT levels. For example I have dropped/changed my dosage many times. Last time from 180mg to 110mg and it wasn’t hard. Dropping only 20mg and he will probably have to drop more in 6-8 weeks time and go through another hormone flux (that usually makes your feel off). But of course everyone is different. And no one better than Dr Saya to determine changes in protocol.
My opinion is that people should stop chasing numbers and have the idea that the more is better. I know you agree with that. So many people on TRT aim for very high numbers (a lot of them with the simple goal of building muscle). Nothing wrong with that but then it is no longer TRT but yes a mild cycle.

Thank you so much for replying, Dr I'll decrease the testosterone to 56mg twice a week. Would you like me to change my thyroid protocol before our next consult as well?

Timing of labs (regarding timing of taking thyroid meds) and variable absorption are taken into account when considering thyroid dosage changes along with the most important variable...symptoms...the apparent lack of which would lead me to advise you to hold steady for now on thyroid, change T cyp dosage, and re-assess. This would also offer the often elusive benefit of only changing one variable and, consequently, the ability to attribute any subjective/symptom changes accordingly.

I've never experienced such a situation as I've just seen here. A patient has a medication problem so he goes to an internet discussion forum to look for help. Then, his own doctor arrives at the same forum to adjust his meds. So, this is how it's done?

Just so you understand Dr. Saya has always been part of this forum and deals with his patients at his office as he is the lead physician at Defy.

Some patients may have issues that come up and are not able to see Dr. Saya right away or one of the other doctors which work under him.The poster tarnation was looking for advice/guidance from forum members regarding his concerns as his next consultation with Dr. Saya is still a few months away.

Looks like his doctor had time to respond and what just happened was truly awesome!

Thanks. I know who Dr. Says is. I just don't understand why you would wait for months for the next consultation when all you have to do is pick up the phone now. It looks like tarnation finally came to that conclusion part way through the thread. Tarnation just seemed a little desperate to me, and I kept wondering what was stopping him from making a simple call to the doctor, instead of waiting for months for some resolution. I can't imagine anyone would adjust their own meds based on forum feedback. Peace.

We never advise that anyone adjust their meds on Forum feedback independently of their doctor. We do, however, offer suggestions that can lead to more fruitful discussions when a member engages with their doctor either on the phone or in an office consultation. The collective experience here doesn't substitute for a physician's guideance, but can prepare a member to become a more informed advocate for their own health.

Dr. Saya being an active presence in the forums and offering particular, specific and useful advice is what got me to start with Defy. It is unusual and appreciated. Welcome to the way medicine can be done collaboratively. Thank you, Dr. Saya and all the forum members who have so much useful advice.

Are you a patient with Defy? You can't "pick up the phone" and talk to Dr. Saya. You need to schedule a consultation with him, which often times takes several weeks if not months. Even talking to a registered nurse has an estimated wait time of several weeks at the moment. The reason I came to this forum is because there are so many people here who are extremely knowledgeable about TRT / Thyroid issues. If it wasn't for this community, I would probably have never even gotten diagnosed for hypogonadism.

You seem like you're relatively new to the site / community. Once you get a better sense of the community here, I'm sure you won't find it crazy that I value the opinions of several posters here very highly.

Good response, you don't always need to used Dr. Saya for everything. I now usually use one of their nurses for adjustments to my protocol.

Thanks, the reason I came to the forum was to get a better sense of why my protocol isn't working. Also, the nurse has an expected wait time of several weeks, and I was hoping to get insight on my labs (from you and the rest of the community) before speaking with an actual practitioner.

If it wasn't for the collective thoughts from this community, I probably would have never gotten my testosterone / thyroid levels checked. I think the community offers an amazing resource that everyone should use in conjunction with a doctor. There are many terrible doctors out there, so knowing about possible protocol adjustments or diagnoses can help filter the bad doctors from the ones worth seeing.

Your testosterone levels are way too high. I bet you have a low SHBG like me. 140 mg a week is producing results that high? Wow.

I am on 114 mg a week and 250 ius HCG and my numbers on Free testosterone are higher.
