Hello all,

I am looking for opinions on some interesting findings in my bloodwork.

A little background: I am 28 years old. 6’1” 215lbs. I have been on TRT for 3 years. My bloodwork has always been somewhat consistent with never any major changes to protocol. I have always been on the same dose of 200mg/wk of Cypionate with E3D injections. Here is a brief summary of my blood level averages for the past three years:

Total T: 850-1150 (264-916) ref range
Free T: 26-28 (9-26) ref range
E2 sensitive: 19-28 (8-35) ref range
DHT: 65-94 (30-85) ref range
SHBG: 23-45 (16-55) ref range
Prolactin: 8-13 (4-15) ref range

So, here is where it gets interesting. My last two blood tests have come back with elevated levels of almost ALL those listed above.

Bloodwork taken on 11/27/2019
Total T: 1485 (264-916) ref range
Free T: 31.5(9-26) ref range
E2 sensitive: 53(8-35) ref range
DHT: 118(30-85) ref range

Bloodwork taken 1/2/20
Total T: 1335 (264-916) ref range
Free T: 65.3(9-26) ref range
E2 sensitive: 48.5(8-35) ref range
DHT: 132(30-85) ref range

Bloodwork is always done at trough. None of my levels have ever been that high at trough. I have never had to manage estrogen at this dose. Although I will say I have taken Arimidex in the past to combat possible estrogenic side effects. Same pharmacy with the same concentration of drug at the same dose. My weight has stayed between 212-220. Diet has remained consistent. No added supplements. No HCG. Exercise daily. Sleep average is the same. Overall, no drastic lifestyle changes and no protocol changes.

From a physical standpoint I feel good. Libido and erections are awesome, nocturnal erections every night, strength and energy are good, mood is good. The only side effect I have currently is a tight scrotum hanging high. Which I’ve never experienced throughout the years with no HCG therapy. About one month ago I didn’t feel this way which is why I got labs done. Water retention, bloating, libido decrease, erection quality decrease, fatigue and poor sleep. I would assume from high E2. I didn’t make any changes. Simply stayed the course up to now.

My question is, why all of a sudden, with no changes, is my body handling these hormones at this elevated levels? My only guess is that for whatever reason my SHBG has lowered even more producing more unbound levels of hormones. Thoughts? Suggestions? Other bloodwork I should have done?

I have personally encountered smaller testicles when blood pressure or hormone levels are too high, once these are lower, testicles hang 70% of normal on TRT. Your body will change as you age, as this happens changes to your protocol will be necessary.

I have seen levels change every once in a while, then revert back to where they were previously without protocol changes. You lost the hormone regulation when you went on TRT and shutdown the HPTA losing the natural pulsatility and is unable to regulate testosterone because you're doing that now.

Back to your previous statement about feeling good and everything else is ideal, I don't see a problem other than your shrunken testicles. Adding HCG to TRT will fix that problem, but you may have to reduce your T dosage a little to compensate for the extra T and E2 that will be produced by your testicles if you choose to add in HCG.

@DRay have you changed the brand of testosterone you’re using?

Your total T went up quite a bit, as well as your free T, so not sure if the possible lowering of your SHBG could be the cause. If that were the case, it explains the free t going up, but not the total T increasing. It’s crazy that your free T doubled in your January labwork, compared to your nov bloodwork.

I’ve being going to the same pharmacy for three years and they have been supplying me with the same concentration at the same dose. I suppose it’s possible that they changed sources or what oil it is mixed with?
 
When I asked my doctor about the elevated levels, he didn’t have an answer or an explanation. He only suggested that I was taking something else along with TRT. Which, if I was, then I wouldn’t be asking him why my levels were elevated nor would I be posting on this forum asking for opinions.

Good point lol. Hey doc, just wanted to rat myself out and let you know I’m doing additional compounds behind your back
