Description​

Introduction

The Penuma ® implant is a medical-grade silicone device for cosmetic penile augmentation. Reported removal rates range from 3–11%, most often due to infection, pain, or patient dissatisfaction.


Objectives

To demonstrate a systematic, safe, and reproducible surgical technique for Penuma® implant removal.


Methods

Using the original incision, sharp and blunt dissection is carried down to the level of the capsule. The proximal end of the implant is identified via sharp capsulotomy. The penis is everted to expose and dissect the distal attachments. The mesh is removed with superficial sharp dissection, avoiding cautery to protect the neurovascular bundle. After complete removal of all permanent material, hemostasis is achieved, a drain is placed as indicated, and the incision is irrigated and closed in multiple layers.


Results

This video demonstrates a safe and feasible operative approach for complete Penuma® (Himplant) removal while minimizing risk to surrounding structures.


Conclusions

Penuma® implant removal can be performed safely and effectively using a meticulous, stepwise approach. This technique may reduce complications and preserve penile function in cases requiring device explantation.
