High prolactin and low T

Guys,I'm asking for a friend,posted on FB group too.

He's 23 and has low normal T (400ng/dl).

Blood work showed high morning prolactin (700 on range 88-330). AFAIK,high prolactin can cause low T by downregulating LH/FSH in the pituitary gland so testies don't get signal to produce. But his LH/FSH is high normal. Also no symptoms of elevated prolactin (vision,headache,ED).

Would lowering prolactin increase his testosterone significantly?

From the sounds of it, depending on what you call high normal, no. Sounds like your friend is probably primary, which is interesting that the prolactin didn't suppress LH and FSH more, so without that I imagine LH and FSH would be sky high.

Post the full labs, I'm really interested.

That was my first tought too. But he's not really hypogonadal,it's low normal level?

I don't have his lab numers,but he said that everything was in range except high prolactin and slightly elevated cortisol.

Did he run these labs himself, or is he under the care of a doctor? If the latter is true, what was the response from his physician?

I doubt it, high or elevated Prolactin seems to show itself with ED and orgasm problems that I know if. If his Prolactin is indeed that high it could be indicative of a pituitary tumor.

Plus if his LH is indeed high normal, then it's may not be a prolactin issue, although I do remember reading that prolactin can inhibit testosterone synthesis in leydig cells, but I don't know how true or how much of an effect it has.

Chances are that low test or low normal test with high LH means primary and lowering prolactin will only raise LH without having much of an effect on testosterone levels.

He sent me these results:

He checked that first and went to endo. Endo said that's normal (is it?),and only after saying he feel the symptoms of low T,endo checked few other hormones:

TSH: 3 [0.4-3.5]
FSH: 9.68 [1.5-12.4]
LH: 7.41 [1.7-8.6]
Prolactin: 700 [88-330]
Cortisol: 602 [171-536] ---->said he was very stressed before bloodwork


Endo told him to do MRI to check for adenoma.

I had numbers similar to that, high normal LH, about 350-380 total test, but here's the kicker 8 E2 7-43 range. My TSH, and prolactin were fine tho.

Yeah I had super low E2 and there was nothing my body could do to fix it. What an absolutely miserable feeling I had all the time, mentally, emotional, and physically.

We don't have sensitive test available here,only standard. I doubt he has low E2 with such high PRL,but it could be possible.

Why would high prolactin stop low E2?

Yeah it's the only way to raise E2, my DHEA-S was top of the range. My T/E2 was about 50. On TRT it's about 30 now.

I don't know,I'm wrong about that probably.

His endo is just not competent IMO and I don't know what to really tell him about that.

Oh, I don't honestly know, I figured you had some research on it or something. I would err towards it being incorrect.

I'd tell your friend to come on here, post his info, labs and such and the members here will explain that if that is the case. It sure sounds like it.

Well,problem is,even his basic english sucks,so I'm asking for him. Especially medical terms correlated with TRT. He wouldn't understand anything lol

well then post all his labs, his stats, and ask him his symptoms. No problem being a translator.

That's all he checked,what I posted already,his endo didn't ask for anything else (bad endo IMO).

His symptoms are just as mine before I started TRT. Low energy,depression for no reason,poor sleep,etc

But I'm self administrating,which I can't advice to him.
