Hello all-
I've been doing a lot of online research to get to the bottom of my situation. My situation: 48 year old male, 6 foot and currently 184. (I've been trying to lose the mid section fat, but I was never really overweight too much. Before I was hovering around 198ish?). I am active and work out many times a week-various P90X stuff, weight lifting, lots of hiking. No issues currently with ED, still get the morning wood, sex not as frequent with my wife. (Still will try around 1 time a week, used to be 2 or so, but we have several week dry spells) Working out is frustrating-not a lot of gains, hard to get that fit look

Here's my current relevant lab work: (Labcorp values)

TSH 1.440 uIU/mL 0.450-4.500 DV
Sex Horm Binding Glob, Serum 74.0 High nmol/L 16.5-55.9 DV
Free Testosterone(Direct) 5.0 Low pg/mL 6.8-21.5 BN
DHEA-Sulfate 65.0 Low ug/dL 71.6-375.4 DV
Estradiol 5.8 Low pg/mL 7.6-42.6 DV
Testosterone, Serum 589 ng/dL 348-1197 DV

ALT (SGPT) 24 IU/L 0-44 DV
GGT 68 High IU/L 0-65 DV
Iron, Serum 66 ug/dL 38-169 DV
Cholesterol, Total 209 High mg/dL 100-199 DV
Triglycerides 90 mg/dL 0-149 DV
HDL Cholesterol 58

I had these done back in 2014 and all of the above were right on the borderline of low, but now they have gone to the official "low" levels on the scale. I didn't have the SHBG done in 2014. My total T was the same in 2014-589.

So my issue appears to be very high SHBG & it's binding my T up? Any other insight? I am going to try many of the supplements out there that have been recommended.

It's hard to find a doctor who looks at all the moving parts! I'm in CO

I have to agree it looks like your SHBG is causing your low testosterone. How is your liver? do you drink alcohol? do you eat enough omega 3?
High SHBG can indicate liver disease

No medications. Never done any steroids. Started a supplement regime recently & will provide a list soon

I'm taking a multi-Garden of Life

Recently started: Zinc, D, Boron, Magnesium oil, B, Iodine, Adrenal one, trace minerals

Might be overkill, but I'm doing this for a month to see how things go. I will scale back to a few key ones

I have been a craft beer drinker the past 10 years, but only a few a week. Sometimes I will have spells where I have more a week, sometimes I don't drink any for weeks at a time. I am currently off. I've had my GGT flagged high over the past 8 years, but my recent test has it back in the normal range. I've been taking an omega 3 supplement, but recently cut that one

I just looked, my ALT was normal (24) 0-44 , my GGT was slightly high (68) 0-65. In the past, my ALT was always normal but the GGT was much higher. Also, triglycerides are now normal at 90, but in the past they were elevated. (up to 270's 8 years ago, but declining each blood test I get) Perhaps my liver is messed up

What type of doctor are you currently with/seeing?
Have you considered contacting a TRT specialist?

I am currently with PrimeBody and know that they will help you address all the moving parts as you described above and get you dialed in.

No doctor currently. I've spoken to a few local options and they seem clueless. I made contact with Defy today so I'm in the process of going through them.
