Everything in range except E2 and LH almost at upper limit

Hi all.

I am a 40 year old male with the usual symptoms like lower libido, joint pain, blurry or noticeable lower vision, very low energy and muscle strength and generally just feeling a lot dumber than before. It started to get really bad about a year ago and now my willpower is nearing its end so I need to do something as my body is starting to feel kind of depressed.

As my symptoms seemed testosterone or estrogen related and having sort of a tenderness or pain in my right testicle, I decided to test it first and see if I can confirm or rule it out.

Most come back within range (albeit some nearing the upper limit) except for my estrogen.
Now, even though this is not a sensitive test (don't have those here), it does not seem that much lower than the bottom range but it could explain some symptoms I guess.

I also noticed my LH is almost at the upper range and is kind looking odd compared to the others who are in the middle range. Maybe it could be a indicator that one of my twins is going to the light?

My memory and work capacity sucks at the moment and the doctors in my country are not willing to help so far so I hope you guys could give me your thoughts on my bloodwork as I don't really know what to look for and don't know enough about this stuff to explain the symptoms or see any patterns.

Its in Dutch but I think most of it is the same but I will translate it if needed. I reckon I missed some tests but this one was around 300 euro to do so I tried to get the main stuff in.

Looks like hypogonadal estradiol, which aligns with your symptoms, and which would cause the pituitary to release more LH as it tries to raise estadiol. Testosterone is actually pretty good, but you aren't aromatizing enough.

If you want to stay natural and just do something OTC to bump up E2, you could try DHEA. Beyond that, you might look at hCG monotherapy, or TRT with or without hCG added. If you don't aromatize enough on TRT alone, you can actually supplement estradiol directly. It's rare, but some men need to do it.

So maybe primary or a form thereof?

To be honest, I feel the numbers are not that way of compared to others that have higher LH or are primary. Most of them also have a bad FSH and/or testosterone. Wouldn't this also reflect in my testosterone which is pretty good like you said?

Then again, I don't really know how off the numbers really are with my untrained eye and my e2 is probably lower as its not sensitive but still, I don't want to mask a underlying condition or true cause that will bite me in the ass later. For me, its still a potential right now but no way a certainty and TRT and the like is too big of a journey to just try it I think.

Good thing you mentioned DHEA as I totally forgot about that. Is it possible that my DHEA is just too low or would that reflect in the other numbers? I do read it could cause all of the same symptoms as well but I did not test it as DHEA and DHEA-S is quite expensive to test but I guess I will have to to be sure. Do I have to test both or is DHEA-S enough to go with?

Anything else I could do to narrow it down a bit and be more sure? I have not much to go on right now, let alone be taken seriously by my doctor.
