Hello guys. I am new to hrt as i have only been taking test for 1.5 months now. I started feeling really bad, no energy, low libido, weight gain and all over a year ago. Ive always eaten healty and exercised to take care of my self but have been gaining lots of fat around my midsection bad last couple of years. My wife and I had fertility issues starting 6 years ago and i had my test checked then, when i was in good shape and was 30 years old and the doc said it was "normal". I just recently found out it was like 375 then. My last test was a 220! So my primary doctor prescribed me a test cream that i have been on for 1.5 months now. I immediately started feeling better but after reading posts on here and watching some of Nelson's videos, realized that my primary doctor isnt the right person to go about getting hrt. I have searched for docs around me, but all of them want me to come to them once or twice a week for injections. So i have ordered hrt to self medicate, bc i cant drive 45 mins 2 times a week for shots with my job. Anyway, i recently did bloodwork so i can order my hrt and my results are crazy! My total test went from 220 to 1465 and free test from 4.2 to 20.0 with my 5% cream. My estradiol levels are what concern me the most though. It was <5.0 and flagged as low. The test that was used was the Roche ECLIA methodology. Is this something to worry about or will it come up after a while? Do i need to try a different test to check it? I am only 36 so i definitely want to do this the right way. Any suggestions or help would be much appreciated guys!

The Roche ECLIA methodology test is for women. It can not accurately assess the Estradiol levels in men. You need the Estradiol Sensitive LC/MS/MS assay.
 
If you still want to remain fertile I wouldn't be injecting testosterone. You should probably try to restart your levels by using low dose clomid.
 
Fertility doesnt really matter bc we had to freeze sperm for IVF to get pregnant. We have plenty in storage. But what is my chances of jump starting my natural with the clomid? Im all for natural if it will work. I just want to feel better if possible. I've been working out hard to get back in shape and im not too bad in weight now. Im 6'1 and 208 but most of my fat is inner thighs lower stomach/love handles. I definitely dont have as much muscle definition as i did several yrs ago even though ive always worked out. I just want to get back to feeling normal.
 
Any low E symptoms? Barring lab error which can occur I'd retest that and no that's not the right test, you need the (Ultra) Sensitive LC/MS/MS test.

Given its a daily cream I don't see the point in testing SHBG at this time.
 
No low e symptoms just wondered why it was low. The only other conserns on my entire blood test was my ldl was 98 which is at the upper end of range and my BUN level was 20 which is upper end of in range too

T3 uptake and Free Thyroxine index provide no real diagnostic value, these tests are typically ignored by up to date doctors. Free T3 is the most potent thyroid hormone which speeds up every cell in your body and drive fat burning ability and your doctor thought why not just skip it.

Doctor could have skipped T3 uptake and Free Thyroxine index and spent the resources on testing Free T3 which provides more valuable information. Reverse T3 can block good Free T3 levels at the T3 receptors since they both compete for the same receptor.

Testing antibodies is also a good idea, there are a lot of people walking around with hashimoto's and don't even know it. A TSH closer to 1.0 is optimal and a TSH of 3.1 isn't. It's showing that the thyroid is requesting more thyroid hormones, if the thyroid doesn't respond to the stimulation, TSH will continue to increase and symptoms become more noticable.

If you are on TRT, I am not sure clomid would do anything for you as long as you are taking exogenous testosterone.

Clomid blocks the hormone estrogen from interacting with your pituitary gland. When estrogen interacts with the pituitary gland, less luteinizing hormone (LH) and follicle-stimulating hormone (FSH) are produced. This leads to a decrease in testosterone and therefore decreased production of sperm. Because Clomid blocks estrogen’s interaction with the pituitary gland, there is an increase in LH, FSH, and testosterone in the body.

When I did take Clomid, it increased both my TT and E2, but it was more effective in raising E2 than TT. I didn't feel anything from taking Clomid, but my TT only got as high as 478 ng/dl with my LH being at 9.4 which was a little over range. I was not on exogenous testosterone at the time.
