Guys...


Could use some thoughts...my progress has been great. A lot more energy in general, been feeling good overall but have an issue.


I am 37 years old. I have a hard time reaching orgasm during sex as many times i feel desensitized. I have read through the forums spoken with my doctor and haven't gotten a great answer.


Most of the time i am able to get hard but many times i am not able to finish and can just keep going.


I just had my most recent blood work done on a trough right before my next shot was due. I was doing .25mL EOD Test Cyp and 250 IU E3D of HCG.


The results are as follows:


Luteinizing Hormone - <0.1 IU/L
Prolactin - 4.8 ng/mL - Reference Range - 4.1-18.4
Progesterone - 0.2 ng/mL
Testosterone Total - 492.2 ng/dL - Reference Range - 264.0-916.0
Testosterone Free - 22.8 pg/dL - Reference Range - 8.7 - 25.1
Estradiol (Ultrasensitive) - 26 pg/mL - Reference Range - < OR = 29
DiHydrotestosterone - 39 ng/dL
SHBG - 14 nmol/L


I recently switched to an every day protocol.


For 10-12 days doing .12mL daily TC and still doing 250 HCG MWF


I am still having issues with desensitization and more recently (last few weeks) even maintaining an erection which is really getting depressing.


Doctor suggested lowering dose to .9mL daily and possibly doing HCG daily at 120iu per day or upping dose to 500 MWF.


I am starting to be at a loss but i am 37, in pretty good shape. Prolactin still a bit low at 4.8 which has me a little concerned...i think my DHT is ok so dont know what to do. Should i be checking e2 every week?


Any thoughts on this?

I would suggest that your Total T is too low. I think most here get to approx. 800 or higher. Some of us can go lower but only if there are no sides. Also progesterone looks low and what about your DHEA? I had the same issue as you and getting my DHEA and Progesterone higher along with a higher dose of HCG helped big time. Thyroid can play a big part in this too but you haven't posted any labs for that.

Getting your LH lab is pointless as you are on test already so this means whoever Rx'd your labs doesn't appear to know this. I'm suspecting your MD is not in the know. Lowering your test when it's only 492 seems incorrect as well.

That was my test level on a trough...don't know DHEA and don't know what progesterone level should be

From when those labs were taken I've now switched to a daily protocol ...don't know what to do from here. That's the problem...was it low bc of dose or SHBG
 
With a low SHBG it could take quite awhile before you have your full erections back, my SHBG is also low (18) and I'm still noticing improvements 7 months later. It's far from 100 percent, it can take up to a year for erections and sensitivity to return.

This is true, I have a lower SHBG and am approaching 1 year on TRT and the last 2 months have given me the best results thus far.

My erections and sex was good for a while and then got bad and goes back and forth at times to the point where I feel like I'm playing tug of war

Please... Don't get me started on desensitization. I used to be a 2 pump chump and had to think about "Kevin Spacey's head floating in front of me" (if you are a fan of Gavin McInnis) to hold out. Since TRT, it's like 30 - 40 minute affair to reach climax, and instead of a slow buildup, it just sneaks up on me... But WOW! they are spectacular.
What other meds are you on? Any SSRIs? They tend to desensitize (They are actually prescribed for guys with premature ejac).
My doc said the full effects for LIBIDO on TRT could take months to take effect.

Also, STOP masturbating if you are still doing this. That's the first line of advice my doc gave me. I also have a hooded bandit, and I've found that retracting my foreskin helps a lot.
Your ED problems, I would say, are psychogenic. You're thinking about it too much. Maybe add 5mg of daily Cialis to your protocol? Sounds like a little jumpstart (it will do wonders for your mentally induced issue) may help.

I'm on crestor (lowest dose) lovaza and metformin. No SSRI. Also I'm young still
 
test and dht is in the normal range but couldnt hurt to raise a little. to be honest i really dont understand injecting a long ester everyday. I know some guys here like to do it, but it just makes no sense.

In the 4th week after starting TRT I had an erection last for half a day and the sensitivity was insanely high, then at the end of every week it would happen again for 2 months, then it stopped. It's a tug of war, it gets better, then recedes, then comes back stronger only to recede again. It quite distressing and creates a lot of self doubt if one is actually improving at all. I remember reading this in another forum somewhere where a guy shared his experience of this tug of war that happens when one starts TRT, it brought me immeasurable relief to know that this tug of war eventually wins in your favor.

I would love to know the mechanism for why it happens the way that it does!

I had done the same, 1 month on, 1 month off. The whole time my numbers were in line with what they should be, I dont know what causes it to be honest. I feel like the wave I am currently on is going to last for a while.

One thing I noticed, and I think has really helped me out overall is light meditation. Nothing crazy, chanting or anything, just basic breathing exercises. When doing this I found that the relaxation released pressures in my pelvic area aka Pelvic Floor. While doing the relaxing/breathing I felt blood flowing to the penis, did some research and found my pelvic floor was always tight, or seemed to be always tightened. Relaxing of this area has restored spontaneous erections, and even helped relieve stress.


Sleep plays a huge factor as well. As we know T is produced by the testicles while we sleep, so poor sleep will get you poor T levels. But since we are now on TRT and provide our own T, this shouldnt matter, right? Wrong, sleep is still a HUGE factor in libido. Even though the T issue is resolved, there is a lot more to libido that just T. Sleep is needed for the brain to send them love signals to the proper places. If the brain is tired and distressed due to poor sleep, it wont happen. I've got a appointment with a Sleep Study Dr this month, I snore loud since starting TRT and think I may have Sleep Apnea. The wife has videoed my snoring, woken me up and I swear to her I am not even asleep. The strange thing is I dont feel like I am sleeping when this happens. Will be posting results after the study.
