Doctor wants me to sign a controlled substance form for TRT

Good afternoon everyone,my PCP has me sign a controlled substance form,so that he can give me trt.But he wants to give me 1 shot of test a month,I declined ,and now want to search elsewhere for trt.But on the controlled substance form ,it reads I agree not to receive trt from anyone else,what to do?

That clause has to mean that you wouldn't simultaneously receive TRT from elsewhere. It would be extremely unreasonable to deny you other avenues of treatment. But in the worst case you would need to find a new PCP, which would surely nullify the agreement. However, I'm no lawyer, so if you want to be completely sure then you should consult one.

That is a pretty common form in my experience. Even Defy has you sign one. It probably covers their a**es in case someone abuses TRT and law enforcement investigates the clinic. I wouldn’t fret - just sign it.

However it does sound like you need better treatment advice. Try a TRT clinic (I really like Defy) or a urologist.

Hmm, don't remember Defy asking me to sign one. But it has been a while. Interesting.
