Cabergoline first, then Clomid? Secondary hypogonadism + slightly elevated prolactin

Hi all, looking for input on my results and planned approach.

Lab results (morning fasted draw):
  • Total Testosterone: 2.52 ng/mL — LOW
  • Free Testosterone: 13.14 pg/mL — LOW
  • SHBG: 36.87 nmol/L — Normal
  • LH: 4.902 mUI/ml — Normal
  • FSH: 4.213 mUI/ml — Normal
  • Prolactin: 419.1 mIU/L — Slightly elevated
  • Estradiol: 28.3 pg/ml — Normal
  • TSH: 2.748 μIU/mL — Normal
  • PSA: 0.713 ng/ml — Normal
  • Hematocrit: 47.7% — Normal

My interpretation: Normal LH/FSH with low T points to secondary hypogonadism. Slightly elevated prolactin may be contributing to HPG axis suppression.

Age: 38. No previous testosterone or hormone treatment.

Planned approach:
  1. Start Cabergoline 0.125–0.25mg twice a week to normalize prolactin first
  2. Retest after 6-8 weeks
  3. If T doesn't recover, add Clomid 25mg twice a week
  4. Retest after 6-8 weeks
  5. TRT as last resort if above doesn't work

What do you guys think about this plan?
