After 2months switching to defy medical from kings berg blood work is way off .what went wrong.dr saya changed my test injection from .25 to .35 twice a week reduced my anastrozole to.15twice a week added dhea 30mg /pregenelone50 mg 1tablet nightly.my testosterone now is 1290 was 700 free test30.6 .hemoglogin 16.2 hemocrit 48.8.estradiol sensitive 52.6 was in the 30 range. My cholesterol going up now it is at 213 had been going down it started at 240 8 months ago down to 199 in September .LDL 137 non Hdl 162. In September I felt great within the last two months I could feel something was off lost the sex drive and energy.amy ideas? Have a appt with nurse on the 30th but I would like feed back here. I been on a testosterone program started in May .just want to be steady can not seem to dial it in like the testosterone theory but to many up and downs right now thanks mike.

I would not worry about your cholesterol too much as going from 199 to 213 could be within normal testing variances. Based upon the Ratio of T to E, your E2 is fine with your Total T at 1290. Ratio: 1290/52.6 = 24, with the ideal range between 14 and 20, so if anything your E2 is LOW, not high. Low E2 causes loss of libido the same as high E2.

The fact that you are going to have a consultation with the staff at Defy prior to the new year should reassure you. The dialling in can be a challenge, but you have every reason to believe it will happen. I will be interested to see how they react to your testosterone levels being so robust.

Yes I will to like I said I felt better at the level I was at in September .he did also up my dose in HCG to 500 iu twice a week and that was twice as much as I was doing before.What would the dhea and pregenelone do that's the only other thing I am doing different

I also think your numbers look pretty good, wonder why you have a low libido and energy? I would think it would be stronger. Let us know what Dr Saya says.

Your estradiol is quite high regardless of your testosterone level. I wouldn't be surprised if that is the issue. Regardless of the T/E2 ratio theory, high E2 is high E2 is high E2. Since you are having symptoms of high E2, I wouldn't be surprised if Dr. Saya increases your anastrozole dosage to .25 mg twice a week.

While I personally don't think you have high E2 for the simple reason of doing the math, here are some common high E2 symptoms:

Hot Flashes/Body Temperature Fluctuations
High Blood Pressure
Water Retention / "Bloated Face" / Joint Swelling
Low Libido / Erectile Dysfunction / Weak Erections
No Morning Wood
Constipation
Irritability / Anxiety / Paranoia
Flushed Face due to High BP
Lack of Motivation / Lethargic Feeling
Lack of Sweating or Excessive Sweating
Aches and Pains
Over-Emotional - Excessive Feelings of Sadness, Anger, Fear

Besides lower libido - which is common with low E2 as well - do you have any of these?
 
I'm of the T:E ratio theory but 52 coupled with your symptom (complaint) could be in need of treatment, more treatment than .15

E2 management is the most trifling thing with TRT, it's highly individual, you want to make SMALL changes, it's something I think you creep up on with multiple tests over time. You're two months in to it and that just a ripple in time.

Like ERO, I believe the testosterone/estradiol ratio is sensible. Like Vince Carter, I acknowledge that each one of us is unique and holding to any number when measuring either testosterone or estradiol can obscure what is actually going on. Your sex drive and energy have diminished; is there anything else in your life that might account for that? How is your sleep? Your level of stress? Exogenous testosterone isn't an elixir, there will be times you aren't on top of your game.

All that said, you may well be dealing with high E2. But, please, I encourage you to slowly ramp up your dose, if that's where you and Defy go. It is very easy to overshoot the mark with an AI.
