A few questions on hormones and erectile dysfunction

I'm new here. I've explored the website a little bit and found good posts regarding sexual health, which is what I'm inquiring about. I've had erectile dysfunction for 15 years (I'm 32), and I've seen many doctors and therapists over the years. I've also seen two urologists. No doctor has ever found the cause of my erectile dysfunction, and therapists always tell me I'm normal. But I'm not: I never have morning wood, and I always struggle to obtain and maintain an erection during sex.

I think the cause of my erectile dysfunction is porn. I won't go into detail here, it would be too long. But after learning about porn's impact on libido in Gary Wilson's book "Your Brain on Porn", and visiting forums like Reboot Nation and Your Brain Rebalanced, it started making sense to me that my erectile dysfunction might be porn-induced. The thing is, I've stopped using porn more than a year ago, and I still have erectile dysfunction. I know it can take more than one year to heal from porn induced erectile dysfunction, but I've been considering other causes of ED lately, and that's why I joined this forum.

My urologist told me my hormones are fine. He's probably right, but I've heard some people say that many urologists don't always know what they're talking about when it comes to libido. That's why I'm on this forum. I want to know if it's true that I might have hormones deficiency that would cause erectile dysfunction, even if my urologist told me my hormones levels are fine. Unfortunately, I only know the levels of my testosterone (550 ng/dL) and my free testosterone (265 ng/dL). I don't know the levels of my other hormones, but my urologist told me that my prolactin and thyroid were in range too.

If this post makes sense to anyone, feel free to share your thoughts. I don't exactly know where to start, so any help would be welcome. Thanks.
Do you have any ED issues, when watching p***?
 
The fact that you get very little morning wood would indicate that there might be a physical or hormonal issue in play. Do you ever wake up in the middle of the night with a very hard erection?
 
I've had struggles with ED as well man. I have recently gotten into another possible cause other than hormones and porn and the usual culprits. It could be pelvic floor dysfunction. Essentially, for multiple reasons the muscles in your pelvic floor can either be tense, or loose and cause all types of problems. I am starting to believe this is my problem and I have started taking measures to combat this issue.

Low levels of testosterone and more importantly free testosterone let alone e2 will have a negative impact on erectile function/libido.

Having healthy hormones is critical when it comes to energy, mood, libido, erectile function, cardiovascular health, bone health, immune system.

When it comes to ED/libido much more is involved than just TT, FT, estradiol, DHT, prolactin.

You need to post labs with reference ranges.

Although your FT 550 ng/dL is far from low let alone sub-par what truly matters is where your FT sits and depending on your SHBG even with a TT 550 ng/dL you could still have less than optimal FT.

Even though you posted your FT we have no idea what the reference range is let alone what assay was used.

Keep in mind that although TT is important to know FT is what truly matters as it is the active unbound fraction of testosterone responsible for the positive effects.

The only way to know where your FT truly sits is to have it tested using the most accurate assays such as the gold standard Equilibrium Dialysis or Ultrafiltration.

Just be prewarned that many doctors have huge egos and you bringing this idea up to him may or may not crush it. But if he’s willing to order it for you then great ,but if not, order it yourself through the link, DiscountedLabs has their own doctors that approve the test after you’ve paid. Good luck man

Have you had ultrasound of your testicle? U might suffer from varicocele?
 
Start by having your doctor order a panel like this one:

Homocysteine. I've been reading that even a mild to moderate elevation of it can impair eNO production. Also, check serum B12.

I don't live in the US, I'm in Canada, so I don'y think I can order that specific. My plan is to find a urologist who will have a look at my hormones (that is, all the hormones listed in the DiscountedLabs link). I don't know exactly how I'll do this yet, but I have a few ideas. It might take a while, and cost some money, but I think I can do it.
Understand that it's difficult but hopefully the effort to find a competent doctor will pay off with a fuller clinical picture.
 
How is your cadio vascular condition? A well known cardiologist was recently quoted as saying if the heart had an antenna it would be it’s penis, or words to that affect. if that not your problem have you tried Trimix, use Trimix with a tension band, Encore or equivalent. Trim off the loops on the ends for a more discrete look. Place the band over the penis and behind your scrotum, much better than just around the penis. Try different size, I like an 8
 
I’m in good health. I didn’t investigate my cardio vascular condition in depth, because no doctor has ever seem any symptom, so I think I’m fine on that side.

I haven’t tried Trimix, or a tension band. I have great results with ED pills, so I will stick to them until I find the root of my ED.

Right now I’m focusing on finding the way to get my hormones checked. It’s not easy, because my family doctor and my urologist are not interested in looking at other things than testosterone, prolactin and thyroid. I found two private urology clinics, but they won’t tell me which hormones they evaluate unless I take an appointment. I guess I don’t have choice, I have to take an appointment, but I have the intuition that they will do a very basic and incomplete checkup as well.

This process will take a few weeks, probably even a few months. In the meantime, I’m doing hard mode again. In the world of NoFap, hard mode is the approach where you not only stop watching porn and give up masturbation, but also avoid any sexual stimulation few months. I did that for 100 days a few months ago, and even though I didn’t heal from ED, I noticed some change in me: occasional morning wood, wet dreams, and glimpses of libido in my daily life.

One last thing: I have found a physiotherapist who specializes in perineal and pelvic rehabilitation, including chronic pelvic pain syndrome. I don’t think I have CPPS, but I’ve always had some pain in that region of my body after sex (after masturbation too). I also feel the need to urinate very, very often... So it’s worth exploring this avenue as well.

That not uncommon in the least. I had the same experience with my family doctor and that seems to be the trend with most on TRT. Family doctors are such broad generalists that as long as something is "in range" they consider it "normal". In their defense, they don't keep abreast of emerging trends in hormone treatment nor the historical range change of what is "normal".

Now the part to watch for is a doctor who will treat you for low-T, but wants to cut your Testosterone levels only because you're now above "range" even though your symptoms haven't resolved and the rest of your bloodwork looks good. In short, it's not a quick fix and it's definitely not a one-size (dosage/frequency) fits all.

Any word on pelvic floor exersize?
 
I haven't been consistent. Life gets in the way. SOrry I can't report back better but coincidentally I started up again about a week ago and I do think there is a little difference. I'll try to stick with it.
You can do it man! At a minimum mobility work is just healthy AF anyways
