6 months into TRT, sex issues

Looking over your first post where you stated that you were started on a once-weekly injection of 170 mg T and 2 months later blood work showed TT in the 1000s and FT looking great.

Was blood work done at the true trough?

Blood is drawn the day before my next shot, so I should be at the lowest of the week.

If you are hitting a TT in the low 1000s 7 days post-injection (true trough) then I would put money on it that your FT is still on the higher end and regardless your TT/FT/e2 levels will be much higher earlier in the week.

It is critical that you know where your FT level truly sits on such protocol and you need to have it tested using an accurate assay.

I would look into tweaking your T dose and injection frequency.

Injecting more frequently will not only clip your peak--->trough levels but will result in more stable blood levels throughout the week.

You will most likely be able to lower your overall weekly T dose and still achieve a healthy FT level and drop the AI while at the same time lowering your e2,
RBCs/hemoglobin/hematocrit.

Hi all! I just found this forum.

I am in my 50's, I started TRT back in May. I tested at 500, but my free T was quite low, so we started a 170 once a week protocol with an AI the next day. After 2 months, T jumped above 1000, free looked great, and AI was increased. I just did new bloodwork, numbers are around the same, but my RBC is a little high. I speak with my doctor later this week about the results.

One major topic has been that while the TRT has helped change my body comp, and combined with a daily Cialis, I get good morning wood, my libido and my ability to orgasm (or sometimes hold the erection) has decreased this year over time. I'm not sure if it's AI related, if I need HCG, or what, but the past few weeks, my wife and I will start something in the AM and I can go and go for 15 mins and nothing -- it's like I don't feel anything. And most of the time, libido is just a big "meh." Not quite what I was hoping for.

Anyway, I know this is a process, and hopefully we can get it dialed in. Right now, I'm frustrated.

This was a huge mistake starting you off on a higher dose of T injected once-weekly and to make matters worse you were thrown on an AI right off the hop!

You have absolutely no idea where your FT level truly sits on such protocol yet your doctor increases the dose of your AI.....go figure!

Now in post #5, you stated that Update: doc wants to increase my AI

Really?

Until you know where your FT level truly sits on such protocol you are going to be chasing your tail indefinitely.

Pretty sure that cialis will lower e2, so your true e2 is probably higher than you think, assuming you were taking it alongside TRT and AI.

Understanding Your Hormones

Estradiol (E2)

A form of estrogen produced from testosterone. Important for bone health, mood, and libido. Too high can cause side effects; too low can affect well-being.

DHT

Dihydrotestosterone is a potent androgen derived from testosterone. Affects hair growth, prostate health, and masculinization effects.

Free Testosterone

The biologically active form of testosterone not bound to proteins. Directly available for cellular uptake and biological effects.
