TRT bloodwork 1 year on review remotely in USA? is this possible?

Hi

I am reaching out to this forum as I am in England, UK and seeking alternative advice as I am at a crossroads with my normal "average joe" doctor, who has been kind enough to prescribe me testosterone enanthate every 3 weeks for the last 12 months.

I did initially try testogel but this didn't work...

To confirm I've had 3 years of consistent blood tests done prior to starting TRT - Testosterone Enanthate has worked ... but...

Since being on Testoerone enanthate, there have been alot of improvements in general (detailed below), but my libido has diminished further..and I thought the extra testosterone/levels would have increased my libidio.. it hasn't

In Summary : -

Pros: -
Much happier,
More Energy/focus/drive
No more night sweats,
Slight weight loss, a bit leaner.

Negative:
Libido & Morning Wood have diminished.
Balls shrinkage (I am not worried about fertility)

I have tried to do my own research, I am 47 years old, my doctor is sympathetic but is not a subject matter expert.

I would prefer for a doctor or a subject matter expert in the USA to review my 4 years of bloodwork who specialises in TRT, so it's a simple case of just reviewing my results - that's it.

Is it possible for my bloodwork to be remotely reviewed by someone in the USA?

This is a terrible protocol that is more of a hormone rollercoaster than an effective treatment. Your average joe doctor isn’t doing you any favors. You can’t dose enanthate like this and anyone prescribing enanthate this way gets an F in pharmacokinetics.

You need at a minimum once weekly dosing, 100-150 mg.

Slightly longer but still relatively short-acting agents testosterone enanthate and testosterone cypionate require injections every 1–2 weeks due to half-lives of 5–7 days, respectively. All of the short-acting agents are plagued by a “roller coaster” effect by achieving supraphysiologic levels within 2–4 days after injection followed by sub-therapeutic levels by 10–14 days. A rapid decline in serum levels around 10–14 days has been called “testosterone crash” and is associated with sudden recurrence of TD symptoms

I think Defy Medical does remotely consulting, Dr. Saya and Dr. Calkin’s are exceptional doctors in the field of hormones.

Also there’s Balance My Hormones in Dorset and the Men’s Health Clinic, both are in the UK.

I will see if I can find similar information re dosage to get my doctor to reference this in the uk.

The doctor is currently giving me 500mg/2ml (2 vials) every 3 weeks, I asked if I could get him/the nurse to split this into 250mg (1 vial) every 2 weeks but this wasn't possible as getting appointments is difficult (this is not private/paid for in the UK - I only pay for my prescription).

I will try and ask my doctor again, the alternative is for me to do this myself, will speak with the doctor about this.

My main concern is my libido/"morning wood" is not right,everything seems to have improved but this is very frustrating.

1 other question, in USA where do TRT doctors get their "normal/optimal" reference ranges for bloodtests e.g Test, SHBG Estradoi Haemoglobin Prolactin etc from?

My doctor has reviewed my blood work but not passed any comment since I started, but again he is not an expert, just trying to take care of my health.

Your testosterone levels are only good 50% of the time and this is making your treatment much less effective. Your hormone levels need to be in the therapeutic ranges all the time.

Response to androgens, hormones is very individual, no two people are alike. What optimal levels are good for you may not be optimal for somebody else. In healthcare (sickcare) across the world, everyone gets treated the same.

It sounds like you should split your dose into a more frequent injection protocol.

Most UK clinics (and US) standard start protocol being 100mg/week, your doseage being prescribed at 500mg/3 wks = 167mg/week, so you have plenty of Test E to play with. It wouldnt be a bad thing to try injecting x2 or x3 a week. Do the vials have a flexible cap so you can draw off smaller amounts ?
