Hi all, from the uk. 34 years old. starting TRT due to lifelong ED, and only weird thing in my bloodwork is over the range SHBG 60 ( 5-55 ) everything else is fine. I need caverject to perform, so I'm starting TRT to see if low free test is my issue. I injected 125mg on Sunday night

few questions

1/ this is UGL stuff, no doc here in uk will prescribe trt for high shbg, I want to get bloodwork to check this stuff is legit. when should I check it?

2/ 125mg sounds like a decent dose, I was originally going to shoot for 250mg for a bit of a boost at the start, but I realise too much. Is 125mg a week too high?

3/ I have hcg on hand. when should I start that, after 3 weeks?

I have everything else covered, and if I see an improvement of my symptoms in 6 months, this will be a lifelong commitment

Best piece of advice before jumping on trt.....let alone self treating is to make sure you have had full blood work done as this is critical.

You need to have TT/FT/e2/SHBG/DHT/prolactin/full thyroid panel/salivary cortisol (4 specimens)/psa/CBC which will include hemoglobin/hematocrit (very important to know when on trt).

Although your SHBG is high it still comes down to where your TT sits as you may very well still have a descent FT level.

The only symptom you mention is life long ED.....low-t is not always the cause of such as erectile dysfunction is multi-faceted.

If you need caverject to perform there is a good chance you have some degree of endothelial/vascular dysfunction.

spent close to 5 years trying different tests and treatments, had a doppler done, came back fine, no venous leak. I used to watch porn, abstained for like 6 months, no improvement
I'm testing TRT, there are worse things I could do, like get a penile implant
my test is like 16 on a scale of ( 8 -28 )

Finally as useful piece of info.....with a TT 16 nmol/L (461.1 ng/dL) and an SHBG 60 nmol/L than your FT is definitely LOW!

Start low and go slow.....100 mg/week or 50 mg (every 3.5 days) would be sensible.

Wait 6 weeks until blood levels stabilize than have blood work done at trough.

Blood work is critical when on trt!

I've had multiple tests over years, lowest I've seen is 14, and highest ive seen is 21. Highest SHBG reading is 65 and lowest is like 58

prolactin, thyroid all seem to be normal
estradiol, we dont have the sensitive test in UK but looks normal

As already stated blood at the start, after 6 weeks and then every 3 months, but depending on how you are feeling and whether you are adjusting protocol. That was the advice I had at the outset and think it was good advice.

Was also recommended to opt for more frequent and lower dose injections. Every 3 days seems good for me.

Hcg I started at the same time as I wanted to remain fertile, but this proved to not work. 500iu every 3 days.

I also tend to run quite high shbg and taking boron has seemed to keep it lower.

It seems to be a real balancing act but I’d suggest if you find a sweet spot supported by bloods and energy, etc then just run with that. Temptation is to keep playing with different options never giving your body time to adapt.

hcg did not work? any special form of boron?

Just use now foods supplement. Daily.

Hcg didn’t work and took it religiously 500iu every 3 days but had inconsistent brands and did question the authenticity of some of the vials I got from one supplier no longer in business. Back to a reputable brand now with hmg to get fertility back. 1 week in and enjoying having fuller balls than I can ever remember.

yeah HMG is crazy expensive in UK and I don't want to have kids yet, I just want to keep my testicular size on TRT
