Just thought I’d update on my journey on thus topic

Have been on 500iu of hcg every 3 days and additionally hmg 75iu EOD whilst remaining on TRT and 90mg of deca weekly for joints primarily.

perhaps somewhat optimistically was hoping to get the wife pregnant in month one of this new protocol but hasn’t achieved that result yet and didn’t conduct a new fertility test.

So I realise I was probably a little underdosed for a recovery protocol so was intending to up the hcg to 1500iu every 3 days and continue with the hmg at 75iu EOD. TRT to continue as is. I will also conduct a fertility test mid way through month 2 to verify that I have the desired result in my sperm.

Any thoughts / experiences to help validate or refine this proposed protocol?

Another option is to increase hmg to 150iu but that is the more expensive option and would add circa £200 to the monthly protocol cost.
Did you have any luck, I am in similar shoes as you were

Yes, wife was pregnant during month 3. Now 7 months pregnant and expecting a daughter on October 31st.
The fertility test I did showed that I had restored my fertility and so it became just a matter of time.

I've just read this thread and that's awesome news mate!

I too am in the UK. My wife and I have an 8 year already, but we've been trying to conceive again on and off for about 4 years. I'm 42, my wife 36.
So obviously been fertile previously.

Despite my healthy lifestyle, weight training but due to lack of libido and generally disappointing bedroom performances as well as fatigue, brain fog. I went to see my GP. Blood tests confirmed Test was low to mid range (or normal as far as their guidance suggested....) LH and fsh look OK on paper.
She referred me to an endo consultant but knowing we are trying for baby said it's unlikely they'd shoot for trt as an option.

I really thought it would be a waste of time so cancelled the appointment.
I decided as going private (paying to see specialist) wasn't an option also decided to go alone

I went on self medicated trt from Oct last year 100mg test e, then added in hcg 500ui 3 times a week around Feb this year, thinking the hcg would solve everything but still no pregnancy. I felt great in myself. Gym performance, muscle gain, fat loss, more fun sex was enjoyable and doable for us both!

In June added hmg too at 75iu 3 times a week, then someone I know suggested 150uiu 3 times a week.

It got to a point where didn't know what to do. It's getting us both down. Still no pregnancy, thinking a semen analysis was maybe good idea.

Jump to 2 weeks ago now and I've come off my trt. Embarking on clomid for now.
Strangely felt OK on it.

Week 1 25mg clomid ed but then built up to 50mg by end of week.
Week 2 75mg clomid ed
Week 3 75mg Ed (this is where Iam currently) week 4 50mg Ed is the plan.

Iam finally booking a semen analysis tomorrow, (expensive!!) but how long further should I wait before I get tested?

Would it be best to stay on low dose clomid for longer?

Really running out of options guys and really want to be back trt but can't until we conceive.

12.5 mg everyday or every other day. Seems to work best for most men.

I would keep running your current dose. But most men do better with 12.5 every day or every other day.

most of the studies I read and linked on this forum hcg plus hmg or fsh was the best combination to restore fertility. My first week or so I was taking 1500iu if hcg plus the 75 of hmg.

I feel like I've wasted so long now because of that.

ill let others tackle the clomid question, but three months is the minimum time you should have allowed on the previous regime and at least done a hone fertility test, which whilst not 100% accurate might have helped restore some confidence that the strategy might have been working fine.

min the studies the protocols were followed for much longer although totally recognise cost is a limiting factor for many. I had planned on giving it up to 6 months and the hone fertility test was positive after 2.

My wife went for a blood test at GPs but the time was wrong or missed the hormone spike.

I've got a semen sample to do (nhs) that I'm going to do on Monday, I've also order a medichecks private) blood test for myself to check, shbg, LH, fsh TT and FT, hopefully levels improved in bloowork despite how I feel.

I've been mentally suffering with anxiety a lot over the last few months, with work pressure and still no pregnancy and I'm thinking clomid may not have helped with this, I've even left my job.

I really want to go back on my self administered trt, but will take hcg from the beginning. Guessing 1500 ui 3 x week is preferable then., then when fubds do allow to add in hmg again.

Does this sound reasonable / sensible?

Yes sir that’s right. Baby girl due next week or maybe even earlier.

I hope you get the results you are after. I guess I was lucky coming across the research on this forum and took swift action that gave the outcome we both wanted. As a result I can only report positive views on the combination of hcg and hmg.

The increased anxiety is probably due to low testosterone levels, not to mention the Clomid. I've been on test for over 28 years now, and every time I have decreased my dosage, it has resulted in higher anxiety, due to testosterone's effects on dopamine and GABA receptors. At my age and length of use of testosterone, I would never go off, although since you were on for a much shorter time, I'm sure the effects of going off are much less for you.

For what it's worth, my wife and I finally have my 11-month-old perfect little baby daughter. And I never came off testosterone, just added in HCG (500iu eod was best for me) and HMG (30-60iu per day worked for me.) So test + HCG + HMG definitely did the trick for me. Hopefully you will be just as fortunate.

Simply put, your body needs two hormones to produce sperm: LH to stimulate the Leydig cells and maintain high levels of intra-testicular testosterone, and FSH to stimulate the Sertoli cells, which do 80% of the work in making sperm.

HCG is an analog for LH, so that will replace the LH that your body is currently making.

But HCG will do nothing for your FSH, which will go down since you have started exogenous testosterone again.

To keep up your FSH, you will need to inject either HMG or recombinant FSH. Typical doses for HMG or FSH are 75-150iu every other day, although I used 30-60iu per day and that worked very well for me. HMG and FSH are both very expensive, so take as much as will work with your budget.

1000iu HCG three times per week might be a little much for you. It would be too much for me, as taking large amounts of HCG raised my estrogen too much, gave me gynecomastia and anxiety, decreased my libido, and made me feel miserable. 500iu every other day was the best amount to get the job done for me.
