TRT which causes pelvic floor pain (CPPS)

6'2", 246 lbs (111kg) male (with a decent frame).
I have a 3mm L4-S1 protrusion, but my main issue is severe anal/perineal burning (CPPS) that is directly triggered by TRT (also have back pain).
I’ve tried various esters (test p/e/c/nebido) and injection frequencies (daily/eod/weekly/biweekly), but the androgen load consistently causes pelvic floor pain. Sitting and standing still for longer period triggers pain even more.

On TRT for ~5 years, pelvic pain started after 2-3months post start.
I’ve been through the medical wringer: 4 urologists, 2 Pelvic PTs, 3 neurologists, and 1 neurosurgeon, yet the hormonal link remains the most frustrating factor..

PSA level checked, comes in a norm.
Found out that using finasteride and cabergoline provides a good relief (but symptoms come back eventually, so have to use it frequently-like weekly).
Also, once i've tried to stop TRT for short period and seems like pelvic pain went away.
I dont want to quit TRT, but it seems like nothing works out, variuos e2 management regimes/injection frequencies/esters/PT etc..

Seeking Advice: Has anyone experienced this specific "androgen-induced" pelvic floor reaction? Looking for any suggestions, similar cases, or long-term management strategies that don't involve being a medical mystery to every specialist.
