Daniel A. Hamstra, MD, PhD, FASTRO, FASCO, Chief of Radiation Oncology, Baylor College of Medicine, Houston, Texas, presents an in-depth review of rectal spacing devices in modern prostate radiation therapy. He explains how dose-volume relationships drive rectal toxicity and how rectal spacers improve bowel and sexual quality of life.
Dr. Hamstra discusses the results from the SpaceOAR Phase III trial and subsequent pooled analyses, which demonstrate lower rectal V70, reduced grade 1–2 toxicities, and improved patient-reported outcomes. He also examines newer technologies, including the reversible hyaluronic acid gel spacer and the balloon-based rectal spacer, both of which aim to simplify placement and minimize injection-related risks.
He reviews FDA safety data showing major complications in fewer than 1 in 1,000 cases and highlights findings that spacer use may reduce long-term healthcare costs by preventing toxicity-related visits. Dr. Hamstra argues that while not every patient requires a spacer, select populations, including those with bowel comorbidities or functional concerns, may derive meaningful benefit.
