Hello all,

I've done a fair share of research on this but I still don't believe I have my answer. Several years back my doc told me I had low T and unfortunately at that time I didn't do anything about it. Long story short, I came across something on-line several weeks ago that sucked me down the Testosterone rabbit hole. Since then, I got my bloodwork done through Quest (fasted) and wanted to get your thoughts. Below are my results

Total Testosterone- 426 ng/dl
Free T- 53.9 pg/ml
SHBG- 51 H
Hematocrit- 41.4%
DHEA Sulfate- 236 mcg/dl
LH- 3.9 mIU/mL

I included a few other measurements that I see discussed often on these threads as well. I'm 43 years old and found a chart on this website that state the normal ranges for Free T are 44-244 pg/ml. Does that mean I'm good since I'm in range? I'm also in range with Quest but on the lower side as well. My guess is that I could benefit from being on TRT because I'm so close to the bottom the ranges but wanted to get advice before the next steps. I appreciate your help in advance.

No you are not in a good range for Free T if we go base purly of the data available. Many experts say that Free T in the bottom 25% is a sign of hypogonadism. Others studies show risk of developing prostate cancer with Free T in the bottom 25%.

Other studies on cardiovascular risk show 550> reduces risk across the board.

Quartile 1: Total testosterone below 340 ng/dL.

Quartile 2: Total testosterone be-tween 341-438 ng/dL.

Quartile 3: Total testosterone be-tween 439-549 ng/dL.

Quartile 4: Total testosterone above 550 ng/dL.

Of interest was the finding that Quartiles 1, 2, and 3 had about the same risk of cardiac adverse events. It was only in Quartile 4 (when total testosterone exceeded 550 ng/dL) that the 30% reduction in cardiovascular events occurred.

These data showed that a testosterone threshold of 440 ng/dL was associated with increased Framingham 10-year CVD risk in middle-aged and elderly men. Poor sexual performance, decreased morning erection, and loss of libido had an impact on the testosterone threshold for CVD risk. The threshold level was higher in men with sexual dysfunction.
