It's an invasive procedure for starters and one where you can't manage dosage rates as the body metabolizes. So in other words, once the pellets are inserted into you there is no way to manage serum levels like one can with injections or transdermals where dosages can be adjusted immediately to get serum levels into the optimal levels.
Injection seems to be the one sure fire application for best results.
