Hello all, I am new here this is my first post
I am a male 32 years old, been working out for 14 years. The last few years my recovery has been super slow and have not gained a pound of strength in the last few years (while adding few pounds around my stomach and love handles). Then my sex life has been on and off for about a week or two every month then it’s non existent for the other 2 weeks. I went to my dr to get tested and it’s was border line normal so he said no I’m fine (wanted me to go get a sleep study (he owns the sleep study clinic)). I sleep at least 7 hours each night but still can’t wake up, mind is foggy, loss of concentration at work.

Then I go to a TRT clinic and my results as follow:
Estradiol 17 (range <=60)
FSH 2 (range 1.5-12.4)
LH 3.4 (1.2-8.6)
Prolactin 13.9 (4-15.2)
Testosterone 524 (300-1080)
Free testosterone 6.6 (6-27)
SHBG 35.5 (16.5-55.9)

The PA says I am young I should just get on HCG twice a week (keep in mind I have to go to the clinic every time $20 copay) and clinic is open M-F 8-5 and I work 8-5 so it was impossible for me to go.

I have been thinking about starting my own TRT and get checked out every once in a while.

I have done a 3 cycles of testosterone (500 mg a week with arimidix) the mosta recent cycle was 5 years ago.

I need help with opinions.

Welcome to Excelmale. What are your sleep patterns like? There are many, many men dealing with undiagnosed apnea - which can hammer your hormones. Ardoc is right to raise the question of thyroid testing, not simply TSH, but the full panel, ft3, ft4, rt3, and both antibody tests. PSA is a necessary baseline test before TRT starts, and DHT and DHEA would be valuable along with a CBC and CMP. Finally, your doctor ordered the incorrect estradiol test. The reference range is a sure giveaway. You had the standard e2 test, designed for women, but should always use the sensitive, LC, MS/MS, test.

Thank you for the quick responses, I sleep from 11ish to 6:30-7. I have a desk job, I finish work then go lift weights 5-6X a week lift about an hour then core workouts and cardio 30-40 mins. Eat clean most of the time (at least 80%) drink a couple of beers a week. But seems like my strength has been the same For the last 3-4 years. I eat high lean proteins.

I do do think I have had my thyroid checked and it was normal. Blood work is in normal ranges.

anither weird thing is that I feel a gyno under one nipple in the AM and goes away throughout the day

Was your thyroid workup complete? Did it go beyond the TSH bloodtest? What do you feel under your nipple? A lump?

No thyroid was TSH only, and I do have a BB size lump that goes away throughout the day, but if I get poked or something around the nipples it hurts

Your prolactin is pretty darn high that can cause nipple issues but not gyno your E2 is way to low for that.
I would ask you doctor for something to reduce your Prolactin.

Funny that it is so high and you are not taking any thing. That is not normal.
Your high SHBG is why your free T is in the dumps. a 500 TT means nothing except your doctor won't give you a T script that high, its Free T you want to raise. The low FT is why you feel like crap with brain fog and lack of libido.

My story was basically the same as urs. I’m 31 now. It all started when I was 28. Low normal total and below normal free due to high SHBG. If I were you I would start working with Defy to get on a TRT protocol while at the same time still focusing on other blood markers that could be causing problems such as thyroid, b12, vitamin D, DHEA, iron, various minerals, etc. It took me years going through horrible clinics to find Defy. It’s your decision tho obv, I’m just saying what I would do if I were in ur position

I have done 3 testosterone cycles with the arimidix during and nolva clomid after, 4 years ago and it took me 2 years to get back to my natural top strength levels that I had before I started any testosterone. Nipples feeling the same ever since. Should go ahead and start TRT? I am not a fan of needles, thinking of being on trt for ever scares me.
Kidney disease, blood pressure, diabetes runs in the family.

Well then perhaps you should have family members T checked cause high blood pressure and diabetes is common in people with low T. I had astronomically high blood pressure and I have diabetes after I became low T, however my blood pressure is normal now that I'm on TRT.

Based on what you've told us, I am not at all sure you've had sufficient testing to initiate TRT at this point. You certainly need a more thorough thyroid investigation. You didn't have a PSA run. Your prolactin is worrying. It also makes no sense for the clinic you consulted to require you to come in for your HCG injections, as you mentioned in a separate post...the place sounds suspiciously like a T-Mill.

I'd urge you to consult a more capable doctor and see if any answers to the open questions can be found.

what do I look for when’s i am searching for a doctor? I’m in the Dallas, houston, and Austin Texas area. If anyone knows any doctors pleas let me know.

Well managed TRT will actually help prevent diabetes, help get blood pressure within a normal range, and will not influence or cause any type of kidney disease. Having normal/ high normal testosterone levels is one of the healthiest things u can do, which means u are preventing almost all chronic diseases while on TRT, not causing them. Issues that the media link to TRT can definitely occur, but that’s only if ur TRT is not managed properly. It doesn’t matter where u are located. Defy medical takes patients across the country. They are located in Florida and I live in Massachusetts for example. And I don’t work for them or anything. I’m just a patient of theirs that had horrible experiences with two other clinics, and literally could not be happier with Defy. I can’t imagine any clinic or doctor out there being better than them. Maybe equal to them, but i don’t see how a clinic can get any better.
