Sudden changes after 3 years of TRT. High free test and trouble managing E2

Background: I hit puberty early. Put on muscle very easily but also had a bad case of gynecomastia (fixed surgically in my 30's). Was always lean.

4 years ago I had a blood test that showed my testosterone total was around 300 and my E2 was in the 50's. My physique and anxiety reflected these numbers. Went to a respected hormone clinic in the area. Was put on 200mg a week of test cyp, .5mg of arimidex twice a week and .500 iu of HCG three times a week.

I kept at this regimine for three years. Total test always hovered around 1,000 - 1,200 and free test was always around 35 - 40 ng/dl. E2 was always in the 20's. I felt great. Put on muscle. Leaned out. Anxiety gone. All other health numbers are great.

About a year ago the clinic decided that my Free T3 numbers could use some improving and put me on armour (number was always around 3 and they wanted me at 4). I thought great, maybe this will give me a boost of energy and help lose a bit of fat. It was also around this time that I also started taking a probiotic supplement.

Not sure if the two are related, but it was around the same time that my free testosterone went way up (55.74) and my E2 tanked (16). Didn't feel too great. Energy way down. Libido way down. They suggested lowering the test dosage a bit and to take the arimidex as needed (if I feel bloated, emotional, etc). Ended up being around once every 10 days or so.

Last round of testing showed my free test still a little high and my Estrogen shot up to 70.4! So... they suggested we try .5 arimidex every week. We'll see what that does.

Any ideas? Thanks!

Without seeing your full labs, I would consider going back to your original protocol. See how you feel after about 7 week and if you need to make changes. Change one thing at a time, for at least a month. Six weeks is better. Then you can focus on one more change, and so on.

Understanding Your Hormones

Estradiol (E2)

A form of estrogen produced from testosterone. Important for bone health, mood, and libido. Too high can cause side effects; too low can affect well-being.

DHT

Dihydrotestosterone is a potent androgen derived from testosterone. Affects hair growth, prostate health, and masculinization effects.

Free Testosterone

The biologically active form of testosterone not bound to proteins. Directly available for cellular uptake and biological effects.
