SHBG of 25 Low Libido

I am new to TRT. About 8 weeks in. My first 3 weeks I had good libido and erections, but by week 7 for my fist blood test libido was gone. I have been doing daily injections from the very beginning because I started with an E2 level of 41 (non sensitive test) before even starting my first injection. Seemed to me that maybe my body produced E2 easily so decided to just go daily right away and Total Test was only 479.

Protocol:
80mg a week Test Cypionate/Propianate 80%/20% blend from Wells Pharmacy 200mg/ml
Daily injections shallow IM (Tried Subq first week and had lumps everywhere)

7 Week Blood Results:
Total T 986 ng/dl
Free T 22 ng/dl
Estradiol 61 pg/ml (non sensitive)
SHBG 25 nmol/L

Honestly I was surprised my total T was this high on just 80mg. I'm just one of those guys I guess. I ordered some DIM and Calcium D Glucarate to try and get the E2 down. Have not started it yet. My doc wants me to try Anastrozol as a last resort but I have no idea where to start for dosage. Maybe .25 every 10 days? I dont want to crash my E2 with my body seeming to be so sensitive to substances. I know guys like me with a lowish SHBG can have a hard time getting dialed in. How can I increase my free T and also lower my E2? Even possible? I tried HCG at week 6 to try and help with the mild ED I was getting, and after 2 injections of 300iu my ED got incredibly worse. My penis even got cold to the touch and started it started to ache a bit. I immediately stopped the HCG and lowered my Test dose by 10mg down to 70mg for the week because it seems that I may have high Free E2. Its only been 4 days or so and I can already tell a difference in Erections getting better. Not where they were on weeks 2 and three but better for sure.

I am planning on getting bloods with additional values like prolactin and sensitive E2 in about a month to get more clarity..

Thoughts?

Thanks for any advise

That’s how I felt with HCG as well. It gave me horrible ED and my penis felt numb after taking it for a couple weeks. My guess would be it increased estrogen? I also got very irritable on it, I threw it out a couple days ago and already feel a little better. My guess is that your symptoms right now are due to your E2 level being at 61. I would definitely look into lowering that ASAP

1/8th mg or .125mg E3.5D is where I'd start...and start using the correct E2 testing...use both if you can afford it. Think about adding a third test, "Estradiol, Free", if you can afford it.

I presume you trough tested on those numbers so look at it that way that that's your LOW point, you have peak numbers at about 12hours that are higher. My Free E doubles to over the lab range @ 12hrs on just 16mg/D.

Thanks for the tips. So because I’m doing daily injections in the morning should I not be injecting the morning of my blood draw? I get conflicting answers on this all the time. Do you inject on blood draw day yourself?

You get the blood draw done at trough. So with daily injections, wake up, go get your blood tested, and then inject for the day. The blood draw should be roughly 24 hours after your last injection.

Understanding Your Hormones

Estradiol (E2)

A form of estrogen produced from testosterone. Important for bone health, mood, and libido. Too high can cause side effects; too low can affect well-being.

DHT

Dihydrotestosterone is a potent androgen derived from testosterone. Affects hair growth, prostate health, and masculinization effects.

Free Testosterone

The biologically active form of testosterone not bound to proteins. Directly available for cellular uptake and biological effects.
