Penis and scrotum suddenly so tight it's hard to pee

Hey all, I don’t come by here much these days as my protocol has been pretty spot on. 154mg T cyp a week, EOD when I travel for long periods, and ED when I’m at home. Haven’t needed Cialis in forever, and the lady is quite happy.

Recently (like in the last couple of weeks), my scrotum has pulled up to nearly nothing, and my penis looks like it has a rubber band around the middle and has gotten as short as <1" flacid too at times. It gets so small, it gets slightly painful to pee, and then I end up sitting on the toilet for another 2 minutes squeezing more out so it doesn't dribble later. Super embarrassing. And no this isn’t a troll post.

I know atrophy is normal over time, but in the 4 years I’ve been on TRT, it’s been minimal. I can’t imagine that suddenly everything went to about 25% of its normal size, including my dick.

I had issues like this before with high estrogen, and I have noticed lower libido, weaker erections, and shedding a few more tears than normal since returning from my last 3 month trip a few weeks ago. Admittedly being in Europe, beer consumption was high.

Any ideas what else I might try before my urologist appt this Friday? Been drinking plenty of water, no alcohol, and put the smokes (well, vape) aside after leaving Europe. Hoping I just need to balance back out.

If you have trouble of peeing I would think it's your prostate. HCG should help your testicles enlarge or back to normal size.

Just seems so odd that it happened very suddenly. Had it progressed over the last year, it would make more sense as just atrophy and all that - to be expected after a few years.

The trouble peeing seems directly correlated with this penis length/girth shrinkage.

With the testicle size, I have had them pull up a lot in the past when E2 gets high. Upon that lowering, the boys dropped back down again.

Maybe it's a stretch, but I am in Ecuador currently, which is super high altitude (about 3000 meters I believe). My blood O2 has been low, I feel the altitude, and I know that causes all sorts of circulation problems. Part of my reason to just stick it out a bit and see what happens.

If it's only a testicle issue. I would try to get HCG and see what happens.

It’s not always estrogen. There’s gotta be something you’re forgetting to tell us. I had estrogen as high as 100 with no such issues. Not even a drop of water retention. But changing test dosages even slightly can screw things up for weeks.

I was out hiking today, saw your post, and it gave me some time to think of anything remotely different recently. Here goes:

1. Was in Europe (3 month trip) until 2nd week of May, drank a lot of beer in Germany
2. Had to move out of NYC apartment when I returned, landlord was (as usual) being super high pressure, so didn't stop drinking beer until...
3. Temporarily moved to Ecuador (my business is here, so I'm here a few times a year), cut the drinking about a week ago once I landed
4. With the stress of moving, fell back into nicotine vapes. Stopped the same time as #3, so about a week ago
5. Probably due to #4, haven't been sleeping the best, but far better than in Europe (time zones) and while stressed about moving
6. Had a routine call with my urologist's NP yesterday, she suspects stress, and to give it a few weeks. Stress is certainly going down the last week
7. Recently tapered off and stopped an antidepressant (Nardil) I was using for anxiety reasons. I won't get into specifics why I stopped, but psych and I agreed it was doing more harm than good. Feeling pretty good actually, but hard to tell if random tears are from hormones or stopping the med. NP refused to order bloods yet, also because I'm outside of the US.
8. Realized yesterday that I mixed up bottles of T cyp while in Europe...I started with a clear bottle (Perrigo or whatever I've been using the last couple years), messed up and opened a new brown tinted bottle (Perrigo again) when I got home. Since being in Ecuador, and around when the problem began, I've been using the clear bottle that's been open since Feb/March and expired in May. Easy to point a finger here, but not necessarily it still.
9. Switched to EOD back in March, from ED. Have stuck to it, had zero issues while on my trip. Libido was dangerously high and nearly got me kicked out of a hostel haha.

I can't think of much else. I'm inclined to blame low dopamine levels or something like that from stopping nicotine and Nardil (one of the few AD's that affects dopamine), or using T from a clear bottle that's expired and been open for nearly 4 months. FYI I do keep the bottles in the boxes to protect them from light. They always stay in my carry-on so there's no whacky temperature changes like in a checked bag.

This is all I got for now. And I don't think the NP is wrong...might just take some time for my body to even out. But even she agreed my scrotum shouldn't just go super tight in a few days, and it shouldn't feel like my penis has a big rubber band around it inhibiting my ability to pee normally.

Stress for sure. And changing test. Any change in dosage, frequency and type, will throw you out of balance. What’s the concentration difference between the bottles? Stick to one type and do exactly the same thing for at least 6 weeks. Recently I added just an extra 20 mg to one injection (eod) as the bottle was done and didn’t wanna waste it. I broke out with acne and it literally took 6 weeks to that day for things to finally balance again.

I was asking my NP about HCG, and turns out they no longer work with Empower or Hallendale. Given I'm leaving NY, is Defy still a top dog? I worked with Dr Saya for a couple of years, but got fed up with some of the NPs & PAs there. Felt like they were reading from a sales manual, wheres Saya offered advice like a doctor (i.e. not "we have these berries you can try" or "Hey ever think about nandrolone?").

You might look into Adrenergic tone. Adrenal/Cortisol issues can apparently cause sudden constriction. I remember hearing a doctor interviewed who used a low dose of Progesterone for such situations, however other things could be involved so I am just throwing this out as a place to start.
