Hello,

I am currently trying to get my paperwork to Defy, very interested in this subject. When I start TRT which to choose? After reading Dr. Chrisler's book, it seems like the TDs can be a good option. I am concerned though, as I love working out, coaching, hunting ect, I am wondering if TDs can get my level to the top end of normal. Any thoughts? I am 42 and five weeks ago I was at 529 but forgot free T on that test. Now, I just got labs back and I was at 198 with Free T at 4.7? What could have made my levels drop so fast? My libido has tanked and I am tired, unfocused, and I can barely make it through my workouts? Any thoughts?

As to your question, a significant number of men simply don't respond to trans-dermals - I was one of them and have found injections to work very well. But, I do believe that it's a mode of treatment you should discuss with Dr. Crisler or Dr. Saya since some men are very successful with them. As for the dramatic difference in lab values, the possibility of error should always be considered. What caused you to seek evaluation in the first place? Were the symptoms you described presenting issues before the second, low reading, was obtained? Are you taking other medications/supplements?

I started slowing down, hard to get through my workouts, hard to focus, memory, and now labido issues. I am taking a multi vitamin, resveratrol, fish oil, and D3.

Whole fresh food. Nelson's stickies are among the best on the web for nutrition. A hand full of supplements are backed by studies demonstrating an association with better outcomes. Pick six, read labels, avoid hype.

I personally will not take fish oil. Purified EPA/DHA such as Lovaza are a far better choice. Sardines and non-farmed salmon might provide all the Omega-3's you need. Best to assay serum before supplementing chocalciferol, fat soluble and minerals like iron.

You may require higher doses of B vitamins than can be supplied by muli-vitamins which also contain unwanted stuff like folic acid or which may have insufficient potency. Too many supplements are formulated with anti-nutrients like soybean oil. Check the supplement threads here for cognition/memory, sleep, ant-inflammatory and metabolic boosters.

U.S.P grade powders are usually purer as well as less expensive particularly with ALC, NAC, ALA, L-glutamine. Capsules for ubiquinol/ubiquinone, zinc, zeazanthin/lutein, astaxanthin I will always check for carriers, anti-nutrients and will generally avoid retail brands. Costco has both good and bad

For libido your doc will be checking several things including E2. Coupled with Genes NO stack and possibly hCG you should be in good shape.

Thanks for your help, I am in good shape, that is what is so frusterating, I thought exercise would keep my test level high, but there is a lot of factors involved. I am seeing my doc today, I will see what he says, but I have my paper work into Defy.

Basic serum tests will help determine if you are primary or secondary. Secondary, which is more common, means your testes are still capable but not getting the LH signal. If confirmed secondary you'll want to go down the list excluding environmental, drug exposure ect.

It's not always possible to identify why the axis has shut down. Correcting the situation if possible is preferable to going on TRT, One possibility is that your doc will try a restart. See what you can do to boost your nutrient intake to help things along. Probably won't make a difference but I'd be leaving the squeezed fish scraps in the bottle at least for now along with the reservatol in case there is some impurity in there that disagrees with you.
