Hello, I am 55 yrs old. Type 2 diabetic. Started trt in June level was 270. Started at 100 cc once a week. After 10 weeks blood work showed 1200, doctor backed me down to 50. I was getting chest acne so I took it down to 30.
Everything was great until my wife noticed my scrotum has shrunk noticeably. Blood work last week has me at 597 testosterone.
I started asking questions and the doctors office got nervous.
Now I am worried. What should my next move be?

Welcome to Excelmale. Testicular shrinkage is bound to happen when a man introduces exogenous testosterone into his system. It's a "rule of the road." The fact that your doctor didn't know that is an alarming sign. He or she should have instituted concomitant HCG injections, as almost all of us here at EM are taking, in order to maintain testicle function and backfill important hormonal pathways. It's a standard part of TRT therapy. They should have known that. And now that you do, you realize how much you have to learn. Spend some time reading the sticky posts here in the Forum folders. It's your health and you have to be your own advocate. It will be time well spent.

Plus you are no longer even on a replacement dose of Testosterone. You should go back to 100 mg a week split into 2 doses every 3.5 days. Your current dose is leaving you in a "Low T" state. You need comprehensive blood work as well. Not just Total T.
 
What was your estradiol, on the sensitive test? ERO is correct in regard to your current protocol. You are headed for some misery at 30mg a week. What was your workup like, what tests, and the results, prior to starting TRT. Believe me, you aren't the first guy being treated by a doctor unskilled in androgen replacement. In fact, mist of us went through a number of doctors before we found capable medical care.

Your next move is to fire your current doctor, and get a medical professional that knows what he is doing. Way too many red flags here...dosage, no HCG...what else is this doctor going to subject you in the coming months?

Cut to the chase, and call Defy Medical. You need them in the worst way. You deserve competent medical care, and you will receive it there.

will I do more damage taking the shot or not taking it? It may be 3week before I can get hcg

You never stop TRT cold. Take the shot and get your HCG prescription as quickly as you can. As was noted above, you're setting yourself up for some misery with the dose and protocol you're on. Finding a capable doctor should also be on your priority list.
 
If you took your car in to a mechanic that was never able to fix what was wrong and in fact made your car run a little bit worse worse every time he worked on it, would you A) Keep using him going forward? or B) Find a mechanic that knows WTF he is doing?

I don't say that to be a smart ass, but this is exactly what you are dong with your current TRT provider.

The consult is $250, I guess you're paying for your labs? That's about $200 I think.

There's nothing you can do about the consult price, but if you ask your insurance company if they cover "out of state and out of network" providers lab orders you will find out if they will pay for them. That will save you the lab fees.

That can be a lot of money, yes, but you will get competent medical care regarding a very important part of health. Your physician sounds like he isn't even checking E2. You may be causing yourself harm by not doing this.

Of course there are cheaper options. Stay with the doctor you're currently seeing, who clearly doesn't understand androgen replacement. You won't feel your best, your testicles will fail, but, hey, you're saving money.

Im not a Defy patient (I don't live in the United States), but I do have a great deal of respect for their practice. It's not $500 every month, it's surprisingly affordable after you're in the system. Perhaps you'll find a doctor in your local area who understands TRT. It will be a challenge.

Your testicle hurts because it is most likely shrinking. At this point you need Hcg ASAP.
Getting with defy is the best and fastet course as finding someone like them is hard locally and will take longer.
