Hello guys!

I am new here and really hope somebody can help me with understanding whats wrong with me.

I am a 28 year old guy with sedentary lifestyle.

About 4 month ago I started having terrible panic attacks and generally feeling unwell and depressed. I constantly feel hot my heart is pounding (although it has become slightly better since about 1 month), my body temp is 37 during the day and goes down to 36.4 in evening but I still feel kinda hot. I lost 15 kg in 4 month and while I was skinny fat before now I'm really slim. My face is red but my blood pressure is normal and even on the low end more often than not (100 per 65). I have developed neuropathy type electric vibrations and twitches all over my body but mostly in my legs. They come and go but mostly get better in evening. Even my vision feels blurryish but the eye specialists said my eyes are perfectly fine just dry. I am constantly tired and my muscles feel really tight and while I can fall asleep easily I wake up during the night and have crazy dreams all night. Also I always wake up at 6 am like as if there would be an alarm in my head.

My GP ran basic blood test and said there is no big problem , only my iron is low 6.0 with a normal range starting from 12 (my abc is 5.5 though and my haemoglobin is closer to the upper limit), my uric acid is 450 with upper limit at 420, my b12 is 290 (on the low end) and my d vitamin is 27 also lowish. My recent blood test showed my Creatine kinase is also slightly elevated at 210 (less than 170 is the given range).

I decided to inspect my hormones in private lab and here is what I found out (I did it more then once but here are the latest results)

TSH 0.85
FT3 4.6
FT4 16.5

LH 1.8
FSH 4.5

Testosterone total 310
Estradiol 380 pmol!!! (given range 0 - 146 pmol)
Cortisol 8 am 22 pg ml
Prolactin 8 pg ml
SHGB 32

These tests came back consistantly with minor deviations only in this private lab.

I managed to get to the state clinic where they ran these tests again and my estradiol came back as 15 pg ml ( 13 - 60 pg ml) the first time and less than 10 pg ml the second time. Based on this the doc told me I am fine and don't have an endocrine problem and refuses to treat me in any way.

I don't know who to believe anymore. Based on the tests at the private lab my estradiol is really high while the state lab says its low. The only thing I could get from the technical at the state lab is that they use some sort on CMIA estradiol machine to measure the level in blood.

Did anybody ever experience such differences between the tests? I know the estradiol didn't just go down like that because after the first state test I went back to private lab and it came back 380 pmol again! I went back to the state lab and it showed less than 10 pg ml!!!

Did anybody ever have such symptoms? Can somebody give any input on this?

Defy Medical TRT clinic doctor
Testosterone is low, low estrogen is dangerous and can cause osteoporosis. GP's have no business treating hormone deficiencies, you need a private specialists as most endo's fail miserably at anything related to male hormones. Your estrogen is dangerously low and you have a clinical need for TRT, treatment cannot be refused.

It sounds like your GP is strongly against TRT, most doctors are against TRT and refuse to treat men do to the false belief TRT causes prostate cancer. It's a myth and has been taught in medical school for decades, so doctors are scared to death about prescribing TRT.

The only estrogen labs you should be running is the LC/MS/MS method, any other test is invalid. The wrong test can show overly high results do to testing methodology, some things look like estrogen than aren't in the standard estrogen labs. Most of the time low testosterone is accompanied by low estrogen, mostly but not always. Men convert a small percentage of testosterone into estrogen.

Often guys start TRT running the wrong estrogen labs and based the estrogen blockers dosage on the incorrect test, there levels look higher than they actually are and end up crashing estrogen levels.

Cannot tell how much research you have done on TRT.
If not a lot, I recommend you get a few books.
Get a few good books and educate yourself now:
1. Get Nelsons book "Testosterone: A Man's Guide".
2. Get Lee Myer's book "Natural Versus Testosterone Therapy".
3. Get “Testosterone for life - Dr. Abraham Morgentaller “.
4. Get “The Definitive Testosterone Replacement Therapy MANual: How to OptimizeYour Testosterone For Lifelong Health And Happiness” by Jay Campbell.
These books will give you a much better understanding of TRT....

As noted, the ranges need to be added. If FT3 is on a Labcorp method, the usually that's something like 2.0 to 4.4pg/ml (?)... If that stands true then I would ask if you are taking any type of thyroid medication (NDT, CytoT3, Levo T4, etc.)? If that's a Yes or No, your labs are incomplete, as Reverse T3 and antibodies are still needed (TPO and TgAb).

You mention that your iron is low. Again, what is the complete range not just the starting? It would also be good to compare with ferritin and TIBC. If iron serum is in the tank, I'm telling you as the day is long that your thyroid isn't on the right track. T3 will not adequately transport to the cells without both iron and ferritin at decent levels. T3 will essentially pool and one marker to help the talking points is the Reverse T3 assay.

My last iron reading is 6.7 umol/l range is (12.5 - 32).

Did not check ferritin yet.

My TSH is 0.85 miu/l (0.55 - 4.8)

FT3 4.6 pmol (3.5 - 6.5)

FT4 16.45 pmol (11.5 - 22.5)

Still waiting for my reverse t3 result.

Anti TPO is <28 (0-60)

Anti TG is <15 (0-60)

Tsh inh. Antibody (trak) is 0.4 (<1.5)

I would also like to mention that my results 2 month ago were somewhat higher.

TSH was 0.95, FT3 was 5.33 and FT4 was 18.5.
