Low Testosterone, 20 years old. Help ?

Hello,
new to this forum and didn't know if this forum is the one to go for blood tests etc.

My story : going from one doc to another since over 1 year ( from germany ) since my blood tests showed low testosterone. I experience the typical things like low libido, weak/early erections, loss of motivation and especially dizziness, feeling light headed and low blood pressure ( when/after working out, so when blood pressure rises, I'm feeling good ) which may be related to thyroid issues or thin blood due to low testosterone and therefore low hematocrit ? ( 42.9 range 41-53 )

Ran through multiple blood tests over this time which all showed borderline low testosterone and middle normal to lowish lh/fsh. Also TSH elevated all the time. Will attach all pictures and I hope for some help.

PS : yes, the test is still normal range sometimes but it's too low for 20 years old IMO .

Pictures show german lab ranges and names but I think should be ok if you can read the abbreviations.

THX

Lowish LH? Yours is 15. Normal range testosterone? Yours is literally the end of the range!

Pretty clear primary hypogonadism, at least according to that second test, the first one is completely different.

the 15 LH is after a stimulating test. Normal LH on this paper is LH 3,11. There are 3 LH numbers. First one normal LH serum. Second one is 30min after stimulating test and third one after 60min.

Did they test testosterone with that? If not it just shows that your hypothalamus and pituitary respond but not your testes ability to produce testosterone.

Yes. testosterone before stimulating test 2.41 ( 2.41-8.27 ) and after 2.97 so not much of an increase. It was a test with LH stimulating hormones primary.

I wonder with someone your age would (Clomiphene) Clomid be a better option.
Alternatives to testosterone replacement: Clomiphene ( Clomid )

That assumes he's secondary and his testes are able to respond. This simulation test is rather contradictory, he presents secondary but when his pituitary is stimulated his testes do not respond.

It's always a good idea to try to avoid TRT due to age but to do so with evidence suggesting it'd be futile is rather pointless.

At a young age, I would prefer someone to try Clomid. I think rushing into TRT is not a good idea, when other options may be available.

Understanding Your Hormones

Estradiol (E2)

A form of estrogen produced from testosterone. Important for bone health, mood, and libido. Too high can cause side effects; too low can affect well-being.

DHT

Dihydrotestosterone is a potent androgen derived from testosterone. Affects hair growth, prostate health, and masculinization effects.

Free Testosterone

The biologically active form of testosterone not bound to proteins. Directly available for cellular uptake and biological effects.
