Can anyone tell me if I have gyno. And if so, is surgery the only option?

Do not think this is gyno although a better photograph would help -- one with less shadow. If it is gyno then only surgery will resolve it - not exercise and not liposuction.

Appears that you just have weight in the chest area and you have large nipples. When you lay flat on your back and push in on your nipple in towards your back , do you feel a hard cherry type knot?

Gyno is something that you do not like to have.
Right now it may be hard to tell from the picture.
That is less important.
If you would cut it out, that may or may not take care of cosmetics.
But that will not solve your underlying problem.
