Intro 45 male TRT issues

Just got test results back today. Have been taking test cyp. 100mg weekly for a few years now. Current test results.

Estrodial 50
Hemoglobin 16.7
Hemocrit. 50
Test 634
Throughout the last year my Estrodial has been 41, 18, 36, and now 50. Unfortunately my doctor is pretty much only good at billing my insurance, He’s a urologist and my visits are pretty much come down to a hand shake and see you in 3 month, no matter of my concerns. My blood pressure is also 154/93 and I need to fix this immediately.
Any advice about seeking a physician north of NYC greatly appreciated? I would like to go through my insurance if possible, but am not going to put a price on my health.
