High SHBG

Any thoughts on high SHBG?
My latest labs go as follows:

Total T 462 ng/dl ref. (348-1197)
Free T 5.1 pg/ml ref. (6.8-21.5)
Estradiol sensitive 11 ref. (8-35)
SHBG 101.1 nmoll/L ref (16.5-65.9)

My total T was 898 back on 10/25 this test was on 11/10
Taking 100mg of compounded cream daily. I have an appointment with a new doc next week. My current one will only do pellets and creams... No thanks

Could just be a creams thing. How'd your total vary so wildly there?

Have you had thyroid levels checked? That SHBG is mighty high, higher than any I've seen before, and makes me think something else may be going on.

Also, as you probably already know, your dose of test isn't working too well, low total for TRT, low free, and low E2. You must feel terrible.

Hopefully your new doc knows his stuff!

Consequently your Free T is in the basement, most all your T is bound up.

I'm currently on Nature-throid 1 grain 65mg. I don't feel that bad, I've felt worse with higher T. Sex drive has been pretty shitty lately so at least the result explain that. I've looking forward to starting with a new doc and doing injections. I think the cream has just shut me down and I don't absorb it very well. Just curious if this is a common thing with sone people the high SHBG? I've only ever seen people comment on having low SHBG. I hope injections work better.

When is the last time you had TSH, free T3, and free T4 checked?

Thyroid hormones, specifically T3 have an effect of raising SHBG.

Your libido is probably low due to low E2. transdermal is known for it's sudden failure a few months into use.

Everything seems to be normal. I guess I will see what my new doc says.

Definitely not high, and hyperthyroidism is what causes high SHBG IIRC, so maybe check vitamin D?

I appreciate your input! My vitamin D is finally in check it was low for several years but I just had it checked with these last labs and its finally in range 68 (30-100 ng/ml)

Honestly, this is interesting, do you happen to have other lab results? At this point anything can help suggest something, so anything you have really.

With your E2 being so low I'd hate to see what your SHBG is when E2 is much higher.

Yeah that was from the 10/25

This was the first time I ran SHBG and I ran this lab on my own. The test I ran before that a full saliva panel and everything was pretty normal except for the T was on the low side.

So I guess we will see what the doc says next week.

A saliva hormone panel diurnal cortisol, dhea, progesterone, testosterone, estrogen.

That's all wrong for hormones, no one here uses saliva except for cortisol and that's a specialized test. We all use blood.

This was a reply to a questions asked. All my test have been serum, although i did do a saliva panel as well with my last Doc, who I'm no longer interested in working with.
