Time for an update. TL;DR -- I'm in a black hole of complete emasculation with a total T / free T to die for. I had a good run with 5mg Progesterone / 5mg of DHEA per day. Libido was on the rise, and energy levels seemed to be improving. All that came crashing down about a week ago, when I got sick (and I *never* get sick). I noticed joints were aching and that it was actually hard to climb up stairs (usually I can run up 2-3 steps at a time). On top of that, libido completely disappeared and now, I am completely unable to get an erection. First time in my life this has happened. I couldn't get it up right now if my life depended on it. I finally heard back from Defy and had some lab tests run. It's clear my now former practitioner was not tracking things properly. For one, he wasn't testing sensitive E2. What do you expect from a former gynecologist? Further, he skipped DHT last time around. To add insult to injury, he said, "high DHT has nothing to do with prostate issues, nor does high estrogen". Then he gave me an anecdote how estrogen shots cured his father's prostate cancer. Wonderful. The guy has been snorting glue, it would seem. If it's neither DHT nor estrogen, what is it? My father had prostate cancer and died from a stroke. He had high body fat and male breasts (do the math). My hematocrit is now pushing 55, and the former doc said that's perfectly fine, even knowing my family history. Malpractice arguments aside, I got my lab results back today: Hemoglobin: 18 Hematocrit: 55 H Fasting glucose: 90 LDL: 122 (I'm on a high fat diet) H Testosterone, Serum: 1498 ng/DL H Free T (direct): 26 pg/mL (6.8-21.5) H DHT: 490 ng/dL H Free T4: 1.33 Free T3: 3.7 DHEA-S: 233.4 TSH: 1.980 LH: 0.1 (1.7-8.6) Prolactin: 9.7 PSA: 0.4 IGF-1: 185 RT3: 23.7 H E2 Sensitive: 64.9 (8-35 pg/mL) H SHBG: 68 (16.5-55.9) H So the quick down and dirty here is:

I have what I see to be a dangerously high hematocrit, likely because my T dose has been too high. I haven't been drinking much water lately, so it could also be dehydration.
Fasting glucose is pre-diabetic (though I think high cortisol is the culprit here).
T and free T seem to be too high, but with that continued high SHBG, maybe not.
Same goes for E2 -- looks high, but SHBG could make it feel otherwise.
I don't have high E2 symptoms. On the contrary, I have ED, zero desire, and absolutely no nipple or penile sensitivity (none, completely dead to touch). I can't even get an erection with aggressive masturbation or looking at a nude woman. Bottom line, I feel castrated right now. At first, I thought it could be the Progesterone driving E2 too low (which it does in smaller 5-10mg doses). But E2 is higher than the last time here. Makes no sense. The other thing that stands out for me here is the normal free T3 and lower TSH but elevated (again) RT3. And the pattern here is for RT3 and SHBG to go up together. Both are liver mediated. But liver enzymes are in line. So what gives? I'll be talking with someone at Defy tomorrow about this. Hopefully, they'll have a clue what's going on.
