Welcome to EM, please take a breath. Can you confirm your estradiol was the sensitive test, and tell us where you had these labs run - it is an unusual reference range for the e2 test. Absent any more detail, a lot more detail, we can't offer any meaningful commentary. Please help us understand your protocol, what caused you to seek TRT, and some medical background/history. I would not, given how little we know, advise you to stop TRT or change any aspect of your protocol at this point.

You're clearly upset. Help us understand the full picture.

That was a good catch coastwatcher, im so rattled i put wrong reference range for sensitive e2, my labcore test is 56.1 range is 8-35pg/ml, I'm 47 6' around 230lbs exercise 5 days a week cardio and weight lifting, I've had anxiety that has affected my life for over 20yrs, had a break down 8 years ago (nervous)? That started me having panic attacks, was treated with meds on and off years trying to help with anxiety which came with side effects of low to no sex drive..wife tolerated it for years but after 8yrs she had enough and asked me to leave, i get my own place and decided i needed to figure out why my sex drive was so low, went to dr my test results came back between 280-335 tt he tested a couple times, started me on .5cc test cyp weekly number jumped to 780tt, felt great first few weeks, then he bumped me to .75cc weekly number went to 1149tt, i get phone call that my levels to high and to drop down to 1cc every 2 weeks, which i did but i broke it up into .25cc every 3.5 days, lab comes back at 588tt get a call from clinic that im in therapeutic range and to stay on that protocol, so after initial start up i havent felt as good, i started searching internet for help and found this site which has been a blessing to read other mens post and know im not alone, learned about e2 here and asked dr to check mine and he told me no, iasked about hcg to help another no, so after feeling so emotional and like im wanting to cry i went to discounted labs ordered sensitive e2 and it came back at 56.1 range 8.0-35, my sex drive exploded and i was having best sex with wife in years but im on a emotional rollercoaster and i dont know if i should treat e2 try antidepressants or stop trt....sorry for long post im just trying to feel better..thanks for listening

I definitely wouldn't think levels of 56.1 for estradiol is too high but I would like higher levels of T. It does take time to get dialed in, we all must be patient when it come to TRT.

56 seemed to high to me or at least out of ratio, i just feel more physical symptoms than in the past which made me think it was hormonal imbalance, im just struggling right now and trying to get better

Its too high but resolving the issue...terrible Dr from what I can read. That's what I'd work on first, competent medical care because what you've written its' pretty bad. How are you dealing with the anxiety? Counseling, therapy, etc etc

I suspect the problem is not just one thing. Your T and E are not at a good ratio. I would probably start a very light AI dose just to see how you feel and only change one variable.

The hormones are probably exacerbating existing issues though. I would think that is something you have to realize to fix your problems. Good luck.

User joe: that is how i feel is that since starting trt is has made my anxiety worse, which sucks because one reason i started test was i read it can help, which i feel it did the first few weeks, it was like i got my balls back and was feeling really good, then bam i hit low point again, not doing any meds for anxiety, have done some counseling in past, mostly just exercising for stress relief. I plan i labs again in few weeks anything specific i should have checked...im just trying to get where im not walking around in such a high anxiety state while still being able to maintain a good libido

there is a honeymoon phase of trt as we call it around 4-6 weeks in. Don't expect to feel just like that all the time. Once you are dialed in you will get much of those effects though. It takes months to get dialed in so set your expectations accordingly. Your doctor is a moron so you have to get educated and do what's best.

588 TT is a little low for most people to feel best. E2 of 56 is a little too high. I'd personally go up to .6ml/wk and introduce just .125mg anastrozole weekly. I'd probably chop that pill into 2 doses even. You'll feel like hell if you drop the e2 too much. You may adjust somewhat to the elevated e2 so I wouldn't be aggressive.

You will run into more periods of ups and downs until you figure it all out. That's just how it goes.

You didnt list shbg. You need this value as well to get a full picture. Just try to calm your anxiety by educating yourself so you feel in control of this. That's what I would do.

Im just sharing my own opinion and experience here. This stuff varies person to person. Anxiety was an issue addressed with trt for me. Low e2 sets it off for me though. I think I've only seen really high e2 once, and it just made me angry. I don't convert much.

I cant tell you how much i appreciate your help and others, hindsight i wish i would have known about trt before being started on it then i could have had labs done to know where i was starting from..dr told me he's not interested in giving me an estrogen blocker so I'll have to look elsewhere, but it gives me a direction to go so i thank you

Good luck.

Here is a helpful web site.

IMO the anxiety and the TRT are separate issues. Might relate but fixing one won't fix the other, but might help some. For the anxiety do your best to find non-medicated ways to deal with it. For me, it helped to accept that I have it and understand the feelings are just that - feelings, and understand why you are having those feelings and then determine if they are valid or not valid. I call it owning and controlling the anxiety, make it work for you. You have been through a lot so it is expected that you are going to be on an emotional roller coaster. Having anxiety in response to certain situations is normal but it can become too much. Many people have anxiety so you are not alone. Other can comment on the TRT side of things but your dose is similar to mine - I take about 30 mg every 3 days and my E2 was 40 on weekly injections. Have not tested yet on E3D dosing. Also, you might research other things you can do to cut down E2, like maybe no soy, take some zinc . . . maybe other things. You could also just be a high E2 person but worrying about it does no good.
.
One last thing, you said you just started TRT a couple months ago, yet your Dr has made several changes to you protocol already. Others can comments, but I would think a couple months is needed for blood work and T & e2 levels to level out after a protocol change. And, don't place too much weight on one test. Could be an outlier. Slow and steady.

Appreciate your help saul, anxiety has definitely been a major set back in my life, its made or i should say I've let it shrink my world into a little safe zone that im afraid to adventure out of, which is totally opposite of the way i lived before, maybe normal test levels might help maybe not but its worth a shot...

Usually by the time you need testosterone your other hormones need some attention too. It's quite common to have your thyroid looked at very closely and this is not something the avg practitioner does well. I had very bad anxiety for years and I lived in a state of depression.

After being started on many hormones I found that HCG, progesterone, DHEA and T3 solved my anxiety and depression problems completely. It took a while because my MD didn't test for everything initially but as time went on he eventually got to it and when he saw labs low in range for the above he put me on it and it changed my life.

This is why it's imperative that you see a good hormone specialist MD like our sponsor here Defy. They can even do it over the phone and they are not expensive. Your MD has your protocol all wrong and clearly he's not in control.

Your nipples sensitivity is a sign of E2 being too high. I too would suggest in the interim to take a very small dose of AI to help that. Your E2 being at 56 is no cause for alarm but together with the nipple symptoms you have it's probably safe to say your E2 should be brought down a little. It won't take much to do that.
