Should I be worried prior to trt I had psa total below 1 and now the psa total is 4.6 and free 0.4 and free % 9.
Cypionate 70mg weekly.
I'm freaking out because of the low free percentage.

I don't know his outcome but I fear a low free with a high psa can be bad.
 
Step one with PSA is always retest, making sure that you didn't do anything like ejaculate or go for a recent bike ride that would create a falsely high number. Infection is also something to rule out. The same thing happened to me but it came right back down.

What kind of infection if you don't feel sick, no fever, dont hurt to urinate etc.
 
Prostatitis is IIRC the term you want to search for. I don't know too much about it but I know people and have heard of people resolving prostate issues by fixing an infection that you would not have sensed systemically. Dr. Gundry mentioned on one of his podcasts that he resolved his enlarged prostate by fixing his leaky gut issues, and I know of someone who also fixed his issues with antibiotics. The prostate is apparent very sensitive to leaky gut, but whether that would elevate PSA I don't know. I doubt if mainstream docs would know much about this but ad Integrative doc who specializes in such things could be a good option to investigate.

I'll bring this up to my dr. Yes, I think I had sex the night before however not sure lol.
 
Whatever you do don't get a prostate biopsy! This can cause any contained cancer to spread that would have otherwise remained dormant.
Also how would ejaculation elevate psa when I've been married for 25 years with many normal psa? I am sure I've had sex many times prior to psa testing.
