Metformin is the most commonly prescribed oral medication for diabetes. It has side effects, like gastrointestinal distress. Other meds like pioglitazone remain in use by informed providers. Why? Pioglitazone has demonstrated decreased heart attack and stroke events. Pioglitazone gives better outcomes, but also has more serious side effects. It's similar to the ACEI vs ARB question: drugs that decrease cardiovascular inflammation improve outcomes. But they also have more side effects.
