* Despite these limitations, the findings of this review provide valuable guidance for clinical practice. The consistent benefit of combined hCG and FSH therapy in inducing spermatogenesis and achieving pregnancy supports its role as a first-line treatment for men with HH and infertility. Future research should focus on: (1) large, well-designed randomized controlled trials comparing different gonadotropin regimens or other treatment modalities (e.g., aromatase inhibitors, SERMs), (2) long-term follow-up to assess durability of fertility outcomes, hormonal stability, and potential impact on offspring health, (3) standardized reporting of outcomes, including time to spermatogenesis, live birth rates, and adverse events, and (4) consideration and detailed reporting of female partner fertility factors, such as age, reproductive history, and use of assisted reproductive techniques, to better contextualize pregnancy outcomes and minimize potential bias in assessing treatment success.
