Blood work help - High estrogen & high free T

I really appreciate any advice here. I'm looking to proactively manage my TRT as much as possible. My doctor is strict on running labs, I have to request specifically what I want.

- What do you think are the most important labs to run for a 37 year old male on TRT?
- Is running labs quarterly a good timeframe for good monitoring to dial in?
- What's the downside of running Free T this high? (440 pg/mL)


Current regimen: 10 mg of T. Cyp in the shoulder, every night right before bed, with 400 IU's of HCG in same needle as T, but only on M/W/F.

Quest Labs:
Total T = 695 trough (ng/dL, range says 250-1,100)
Free T = 212.7 trough (pg/mL, range says 35-155)
Estradiol = 102 pg/mL (ranges says less than or = 68 pg/mL)
Estradiol Ultrasensitive = 63 pg/mL (range says less than or = 29 pg/mL)

I recently increased my dose to 15 mg daily of T Cyp. and my T & Free T shot up considerably, I have not had my estrogen done yet, but I assume it's gone even higher.

Total T = 1,380 (ng/dL, range says 250-1,100)
Free T = 440.2 (pg/mL, range says 35-155)
Hemaglobin = 17.9 g/dL (range 13.2-17.1)
Hematocrit = 51.5% (range 38.5-50%)

While labs are helpful, the most important thing is how you feel.

Why did you feel the need to increase your dosage?
What do you think are the most important labs to run for a 37 year old male on TRT?
The more important labs for a man of any age are as follows,
  • Total T
  • Free T
  • Estrogen
  • DHT
  • Hemoglobin
  • Hematocrit
  • Prolactin - if ED is present

Thank you for feedback! I increased the dose because I was feeling ok on my regimen, but wondered if I could be feeling better. I noticed an overall improved mood & energy level, but it did come with side effects of increasing my hematocrit, estrogen, and DHT levels. I’ve been losing hair significantly now. Any advice on health hair loss products? I’m trying to find the most natural option, a lot of the hair treatments have nasty chemicals in them that cause cancer or disrupt hormones.

Are there any particular ones you’d recommend I look into that you’ve seen work well for other guys? The company I’m looking at now is HIMS, a topical spray.

but it did come with side effects of increasing my hematocrit, estrogen, and DHT levels.
If by side effects you mean decreased risk of dying, a welcome side effect.
