I don't think insurance will pay if your T is 404 ng/dl. You have to get prior authorization for insurance. You can get a doctor to give you a prescription but insurance is not going to go for it. The doctors that will put you on TRT don't do prior authorization and are cash only. If you want to play in the TRT world you are going to pay cash and get your T from where they want you to. If you don't want to do that you can see an endocrinologist and do a sleep study.
