21 y/o male dealing with low T before and after bilateral varicocelectomy

I’m dealing with weird case of low T. Testosterone currently is 250 and free T is 36.5. I had grade 3++ varicoceles that I got removed this past August. Testosterone was also low then at 222 and 297 for two tests which was on of the two reasons I got them removed. LH is 4.3 and FSH is 9.1. Now I am just baffled because I am a relatively healthy guy(work out daily, sleep well, eat pretty clean) so I don’t know why I’m dealing with this. I am only a little symptomatic with no morning erections, still have a little libido but not raging and it has been harder to put on the muscle in the gym I use to have. My doc wants to get me on clomid 50 mg once a day. Do y'all have any advice or thoughts on that? I want to perserve fertility for at least the next 10 years but I also do not want to get on a medication knowing that it will me more harmful than good. I just know once I get on this it can potentially be lifelong which is a big deal to me.

Fertility can be maintained on TRT, hCG and FSH as well as clomid. Clomid tends to have more side effects, but I wouldn’t say it would cause harm in the short term.

I’ll never understand why doctors start men out on the highest dose of clomid, then turn around and give a guy 50 mg per week testosterone cypionate, which is a below average dose.

So you would say that I could maintain my fertility for the next 10 years by doing TRT and also doing hcg and fsh along with it? Have you seen that clomid can potentially be a long term fix? I would be on it for the next 5-10 years essentially until I have kids is what the doctor said.

That’s what I said. You might be able maintain your fertility on just TRT and hCG

Back to the side effects statement, for some it can, but long term too many unknowns.

Okay thanks for letting me know. Would you recommend me doing a lower dose to start instead of 50 mg each day? Thanks

Understanding Your Hormones

Estradiol (E2)

A form of estrogen produced from testosterone. Important for bone health, mood, and libido. Too high can cause side effects; too low can affect well-being.

DHT

Free Testosterone

The biologically active form of testosterone not bound to proteins. Directly available for cellular uptake and biological effects.
