For those with either NAFLD/NASH or high lipids (trigylcerides, ldl, etc), a drug called mgl-3196 by Madrigal Pharm. just concluded its phase 2 trial and patients had an average of their SHBG increasing 80% from baseline with some over 100%. Drug also statistically significantly lowered...
I think I've made it quite clear that there are plenty of guys out there who seem to not have any medical condition with their low shbg.
With that said, there are tons of studies on "real people" that show low shbg strongly correlated with insulin resistance, fatty liver etc.
I am not trying...
Well you just have all the conclusive answers don't you.
Try spending some time on pubmed. There's tons of studies out there that show how low shbg is HIGHLY associated with insulin resistance, fatty liver, dyslipidemia, and metabolic syndrome.
Of course I can't "prove one or both caused me...
I think there have been enough studies out there to show that low shbg is strongly associated with metabolic syndrome, fatty liver, dyslipidemia, diabetes which are all connected.
That's not to say that there aren't those who have naturally low shbg or have something else going on like low...

Thoughts? I have fatigue and shortness of breath. On Trt - 160mg divided into 2 doses a week. What should I get checked? Iron? Ferritin? Thanks
ERO, were you always low SHBG? Or do you remember a point in time where you felt "normal?"
I have bloodwork showing I once had SHBG well within range a few years ago. But the past 4 years it has been below normal. I was once incredibly healthy and active and whatever has happened to me has...
Im a low SHBG guy (usually single digits).
Do you have muscle loss/weakness?
Fasciculations (muscle twitching)?
Raised triglycerides?
Heat intolerance?
Abdominal weight gain?
Shortness of Breath?
Exercise intolerance?
Any other symptoms?
I think for people with low SHBG, fat mass, ED, and fatigue may already be a problem. I find myself largely non responsive to TRT presumably due to low out of range SHBG.
Atleast from what ive read, ED and fatmass gain on anti-convulsants like Phenytoin is from a decrease in free testosterone...
I'm confused what you're referring to when you say 6 weeks. I've been on TRT for over a year now
I haven't seen any difference in bloodwork or overall feeling in injecting EOD or 2x a week
Looking for more thoughts on what seems to be some kind of underlying health issue
Been with Defy for awhile and have nothing but good things to say about them.
Besides some cognitive benefits of not having low T, I dont feel a whole lot of from TRT. TRT protocol is 160 mg a week split in two doses and HCG.
I used to be a 5-6 day a week gym guy and was in very good shape...
Ok thanks. Could something like borderline hepatic size be the cause of metabolic syndrome/insulin resistance? Given that I have low shbg and atherogenic dyslipidemia both of which pertain to the liver, I'm trying to determine is this simply a matter of giving TRT and the correct lifestyle...
Hey Dr Saya,
So if one has an elevated IGF-1 now and had a low SHBG before even beginning TRT (possibly due to insulin resistance/metabolic syndrome) is there a way of decreasing IGF-1 while on TRT? I'm a patient of yours and physically don't really feel the effects of Testosterone, only...
My original post was edited but it was more of a "have you seen any further research on metformin raising low SHBG caused by insulin resistance". As mentioned in my other posts I do have elevated fasting insulin so it's more of a where do I go from here to correct the resistance and raise SHBG...
But were you taking metformin because you had diagnosed insulin resistance or just because you had Low SHBG in hopes it would raise it ? I thought I remember you being in the latter category but I could be wrong.
Is there a reason you deleted me quoting Nelson? I know he had been interested in this issue and may have learned more knowledge on the question since.
I forget the reference range for SHBG but My SHBG is about 6 or 7 from being in range(whatever the unit of measurement is).
I also have an...
I'm quite positive I'm insulin resistant with an out of range low SHBG and I'm looking into metformin. TRT so far hasn't been anywhere near effective as I thought it'd be and I am realizing that it is most likely (hopefully) because of the low SHBG and insulin resistance.
Hi Dr. Saya, I understand 4 weeks is way too soon to expect results, didn't mean for that to be a focal point of the thread. Just wanted to introduce myself, my current protocol, and see if people who had already been through something similar had any insight.
Im thrilled with the level of...
I know its going to take awhile to figure out just wanted to see if there were any others out there like me who experienced a good amount of weight gain and TRT ended up being the solution and how long it took to feel like themselves again. Going from a very active 25 year old in great shape to...
Any other thoughts on this?
Im trying to figure out if I can attribute my weight gain of 20+ pounds from my norm (weight fluctuated from 180-190 from age 18-26 with considerably more muscle then I have currently) to solely low T and if so, how long itll take to start to feel and look like my...
Yeah my diet is on point, I eat a lower carb diet with a lot of healthy fats and moderate protein. I honestly haven't exercised because I simply feel really weak and don't have the energy. I don't get any kind of pump when working out like I used to. I'm hoping it's just a low testosterone thing
Hey everyone, new to the forum.
Late 20s male, 3 weeks into trt with Defy and Dr Saya. On a 160mg a week twice a week and Hcg 350 mg 2x a week.
So far haven't really noticed anything but wasn't really expecting to.
A couple years ago started feeling awful and have had a host of symptoms. I...
