It may be true that 5 units is likely the minimum that will work for most people, but not all. When I first started Trimix, 3 units would keep me hard for about 4 hours. That was almost 15 years ago. Now I use 13 units for about 2 to 3 hours.
I think your mixture is a little less concentrated than the standard mixture. Anyway, the dose you are taking is not enough for you. Try 25 units next and then 30 units.
Something I don't understand here. I just pulled out my last lab results from Quest Labs. Quest states that the Reference Range for Free Testosterone is 30.0 - 135.0 pg/mL. What am I missing?
My insurance company (Blue Cross) will pay for testosterone only as long as my T stays below 800. They also pay for labs 100% and doctor visits (although the visits are labeled as general and not specifically for TRT.)

Your course of action should be based on your symptoms. Your total T is borderline low, but many men do just fine with the same T as you. By itself, that is not enough to justify TRT. However, your free T and is quite low, and is probably the more important number.
