Defy has me on 126mg test / 90mg nandrolone per week. The nandrolone is to help with some chronic joint pain issues, and from what I understand 90mg to 100mg per week is a common dose for that particular use.
Also, still maybe worth talking to your doctor if everything else, especially e2, is in a good place. I may be misremembering, but I think I recall Dr. Saya saying that prolactin levels in the double digits can cause issues in some men even if technically “in range.”
Before starting caber, mine was at 26.3 (4.0-15.2). Caber starting working pretty quickly, and brought my prolactin down to around 5.0 on a dose of 0.125 twice weekly.
Have you checked your prolactin levels? I was struggling with a lack of penile sensitivity and difficulty reaching orgasm. I tried PT-141 first and couldn't handle the sides. It gave my insane erections and helped a bit with the sensitivity issues, but then I'd suffer from flu-like symptoms...

It's also possible you have elevated prolactin. I did, and while it didn't really affect my libido, it made it damn near impossible to orgasm. A low dose of cabergoline took care of that. Probably worth getting your prolactin checked.
I have not tried naltrexone. I've read a little about it. It seems to help a fair amount of people, although some of the sides people report are concerning (anxiety/sense of doom, psychosis, etc.). It's worth discussing with my doctor though.
