Personally, I don't even test anymore. Just my opinion, but I think much of the testing on these sites is a waste of time. Testing because the dose is too high or there are symptoms from undiscovered issues...
What's your story and how do you feel? Are you supplementing dhea and were you awake long before the test? You should do more tests.

I take undecanoate and think it's massively superior. I skimmed over what you wrote, and just comparing your experience to mine:
- Far too frequent. Undecanoate half-life is 15-35 days depending. T should be dosed at 1/2-1 of the half-life. I get away w/ once every 2 weeks, which happens...
I've seen correcting thyroid raise total T ~100-200 or so, but that's usually from 200-400 up to 400-500. Never seen anything higher.

that's still hardly any t4. Body makes 100 mcg/day, and absorbs 60-80% of oral t4, so that's a little over 20 mcg t4 hitting your system, or 20%.

I'd check your b12 with your MCV that high. A good b vitamin supplement might be helpful.
And as mentioned above, you probably need thyroid hormone. I'd shop doctors until you find one that'll allow a trial. It could help your energy.

You might also want to look into lipoic acid and/or chromium too. Both otc and might help. Probably harmless anyways.
Low shbg is a marker of high insulin, and, your blood glucose is on towards the high end. You might want to test a1c and fasting insulin next time you do a round of tests, just to double check glucose metabolism.
to go back to your original question - yes, blood levels can fluctuate on the same dose and the same day by a surprising amount.
Consider this:
600 ng/dl = .006 mg/l
5 liters of blood in the body = 600 ng/dl * 5 = .03 mg total in blood.

Just fyi. I've had half a dozen friends feel better from adding in some t4 with their Total T4 in the 5-7 range. Boosting it up to 8-10 often gives them some more energy.
A few other obvious things stick out in your labs:
- you may have an issue with blood sugar. I would test your fasting insulin and keep an eye on your a1c (average blood sugar over time) and fasting glucose. An oral glucose tolerance test (with insulin tested) is the gold standard if you can...
