WTF HAPPENED... horrible coughing fit after injection, thought I was dying

I have been on TRT for well over a year now. I recently received testosterone from Hallendale pharmacy and used the vial for the first time. I had been using another pharmacy that had a sesame oil preparation and empower for awhile as well... basically whatever my clinic sent to me. My first injection from this vial was shallow IM in my glute with a 27 gauge 1/2 inch. I have done this before although I usually go for the thighs or delts with this length needle. I injected and began coughing uncontrollably immediately and it lasted for about 10-15 minutes. I was scared to death.

Was this just a random reaction or should I just ditch the whole vial? Maybe I hit a vein or something although I didn't bleed. POME?

Any advice would be helpful as I am going to call my doctor today as well. Keep in mind I have done well over 100 injections and this has never happened.

Oil embolism. "The use of steroid compounds formulated in oil for depot injection can be associated with acute pulmonary reactions characterized primarily by cough, but sometimes with other associated symptoms."

I believe you injected in a vein. When the oil goes through your lung. It will cause you to have a coughing fit, until you finally cough it out. Some people say it feels like you're dying.

I spoke with my doctor and he though it was the result of alcohol in my lungs (meaning something must have gotten into a vein). His suggestion was to let the alcohol on the top of the vial dry (this is a braindead response but I guess he had to say it), but I'm leaning more towards oil in the lungs. I was happy to hear he didn't think it was an allergic reaction. He said don't hesitate to use the same vial again.

Unfortunately it’s a totally normal response when any testosterone is injected partially into a vein. It’s got nothing to do with alcohol on the top of bottles. I inject IM and this happens to me a few times each year. Sometimes it does feel like you’re going to die, sometimes it’s much shorter and easier. It’s always random. Do NOT lay down while this is happening, it will prolong this event making it worse. Stand or sit upright and KNOW this is a normal side effect that will pass in 10 to 15 minutes. I try to distract myself during this time. You can avoid these with topical creams, but it happens rarely as a part of doing injections of testosterone. I hope this helps. It’s all from my primary care doctor (an HIV specialist).

I use short needles. 1/2 inch insulin needles and try to depress the skin to go IM. I do this to avoid wasting medication. I'm wondering if switching to deep IM as in 1 inch in the quad or 1.5 in the glute would reduce the chances of this happening. It comes at the expense of post injection pain.

If you're concerned, you could just aspirate.

It just means you must have slightly moved the needle after aspirating...only explanation. don't even bother aspirating anymore...because the coughing fits are extremely rare...and I know it won't actually hurt me.
