Dr. Paul Turek, a California-based urologist and a leading figure in male health, recently described a case in which he had been asked for a second opinion. A man and his wife had been trying to conceive for some time - over four years - without success. Presenting with a low sperm count, and elevated sperm DNA fragmentation they had been unsuccessful, both on their own and in the lab: IVF had failed. All reproductive hormones were normal, as were genetic possibilities. A varicocele was repaired...still no pregnancy. When Dr. Turek was contacted and asked to review the case, he found that the patient's sperm count had been steadily dropping, as he wrote, "trailing to zero." Which prompted the question: "What is your ejaculate volume?" Interestingly, the semen analysis showed that, along with a decline in sperm count, semen volume was down dramatically. His veru was blocked. The veru montanum, a small bump in the urethra where the ejaculatory ducts enter the penis was impeded. These tiny ducts can be compressed by cyst, plugged by stone, or blocked by scar tissue. A transrectal ultrasound confirmed that a small cyst was present, blocking semen and the sperm it contained. A day surgical procedure opened the flow and within three weeks a semen analysis showed that ejaculatory volume, sperm count, and sperm motility were normal. Pregnancy followed shortly thereafter. Dr. Turek writes that "life is in, and begins, in the details." The question of ejaculatory volume had not been raised prior to his doing so. Any patient dealing with fertility challenges should give passing thought to both it...and to his veru.
