Until I was a out 45 I had a strong physical drive that would remind me almost constantly in the groin area where I would have loved to have intercourse several times a day.

Now at 49, I still think about sex frequently, but I have zero physical drive behind it. Absolutely nothing going on in the groin area to remind me. I can get erections readily with proper stimulation. My problem is a lack of sensitivity. Far too many times I've had to stop intercourse or even receiving oral because I'm just feeling numb.

I've been on TRT for about a year with Defy (0.23ml subQ 3/week) also injecting same amount of HCG. Also taking 2.5mg Cialis daily.

Any thoughts what could be the cause? Any ideas to combat?

What's your sensitive LC/MS/MS Estradiol at and are you using any at all Anastrozole?

That particular E lab was not a "sensitive" and the true value was likely lower. My E had crashed because I was on Anastrozole (0.25mg 3x/week) but was then instructed to stop altogether on 10/31 so that's the last time I've taken any A.
 
It's a common problem around here people crashing their estradiol levels and then losing their libido. It may take time but hopefully it will come back.
 
So I stopped the A beginning of last November, and I did come back feeling better & decent libido. It's only now - since late December that I've lost the libido / sensitivity again. Is it possible my E2 has crept back up since I've quit the A cold-turkey?

Defy had me go to inject 3x/week over 2 to combat the creeping E2.

Hi Lakecoder, I to suffer from issues with libido and lose of sensitivity. I feel your pain.
I've never crashed my E2 but I have let it get too high.
For me my nipples get sensitive and I would experience small waves of emotion over emotional events on TV.

I can't prove it but in the back of my mind there is more to it than just T and E2 lvls when it comes to sexual sensitivity.

To answer the question, yes it could have come back up. That's what's known as estrogen rebound.
Yes you need labs, and should consider going back on the AI at a lower dose, but yes get labs.
 
E2 might not be the issue. Have you measured prolactin level? Prolacteremia can kill your libido, as prolactin levels are typically elevated following ejaculation to allow recovery and recharge. If prolactin is normal, there is potentially more complicated matter involving brain neurotransmitters imbalance, like dopamine, etc
 
I have labs scheduled (dang why is Labcorp SO backed up??) which will also test prolactin. Probably won't get the results for at least two weeks :(

I don't know where you live but it Denver at my LabCorp I never do appointments they are a joke.
I just campout at the door on Monday morning at 8am an do a walkin. I am in and out in less than 20 minutes.

I've been with Defy since 2/2017 and I'm nowhere near stabilized. I went from an Estradiol crash last September to a big peak now. I am bouncing around, feeling not great for a long period, then a change in protocol, then after some time, a few weeks of feeling good, to back to feeling not great for a long period.

How long a time is reasonable for Defy to work out a stable protocol?

That prolactin (which quite possibly could be part of the problem) is going to warrant some discussion during your next consultation. When (and with who) will your next consultation be?

Wow been there done that. Testim Gel mono before Defy. E2 thru the roof and prolactin followed.



Were you just taking your T only to get those numbers?
 
Possible stress is causing the high prolactin and failing groin area. I have read that prolactin increases when trying to produce more DHEAs. If that is true taking DHEA may help.
