No surprise there! You wouldn't believe half the stuff I had to deal with on my experiences with doctors /endos when I started out 10 years ago.

Regardless of Primary or Secondary, treatment is the same. You're just simply replacing and increasing your testosterone serum levels with exogenous testosterone, usually suspended in cottonseed or grape seed oil.

At 55, unless you are wanting kids, don't waste your time with Clomid.

140mg per week seems high, and, you're right, most guys start at 100mg per week. Personally, I don't think cypionate is the best ester. I recently started daily injections of propionate and am feeling a lot better than I was when I was on cypionate.

As others have said, you probably shouldn't waste your time with Clomid. Age is a factor when it comes to recovery of natural production.
