I just called Empower to ask how these changes will affect us as patients if they go into effect. The rep said, and I quote, "we will still be compounding both HCG and FSH regardless of the classification and there are no plans to discontinue either." She was aware of the reclassification issue...
This new law applies to ALL compound pharmacies-- 503A and 503B-- correct? I found this older thread and wasn't sure if it applied to this or not. Pharmacies like Empower would still be affected, correct?

I have been pushing for FSH for six months and his doctor won't prescribe. She said she'd "consider it" if he was still low once his test levels came back up. We are point blank asking for it this time. I want him to be able to get back on test so we can put an end to this...

Thank you SO much for you input, Dr. Saya. Husband is off T right now and was on it for less than a year, so this makes me have a good degree of confidence moving forward with the HCG+Clomid protocol.

Just want to say thanks Sides for your opinion and advice. We will give the HCG + Clomid a go for awhile. He went from 0 - 4 mil/ml in two months on HCG, so hopefully it'll continue rising. Even at .20/unit, FSH would probably be $400 a month or so for us... so we will give this a whirl for now.
Saw the fertility doctor yesterday and she refused FSH/HMG. She said he is doing fine on HCG monotherapy but increased him from 1500IU/3x/week to 2000IU/3x/week. She also wants to add 50mg of Clomid every other day to his regimen. Sigh.
In the fertility world, they will absolutely not advise...

I do wonder if he should continue the Clomid, though. She said he should discontinue. Most the studies I'm reading for gonadotropin therapy either use HCG monotherapy or HCG + HMG or rFSH. I haven't seen much with a SERM combined with a gonadatropin.
TY Fifty. I was surprised she even offered 1500 3x a week. It seems the standard in urology is 1500 2x a week. Unfortunately, we are dealing with The Cleveland Clinic here- so the docs there aren't as pliable as I wish they were. I think we're going to have to stick out this regimen for 30-60...
I have collected no fewer than 12 studies showing that FSH added to HCG has great results in cases like hubby. I will give her 60 days for HCG only. If his next SA is still zero count, I will insist on FSH. She said they don't prescribe it there, but she also said they don't routinely use HCG...

This is a huge problem that we've had. We don't know what caused the zero count SA. We know based on history that it's unlikely he has complete testicular failure simply because there was absolutely nothing that could have caused it and he had no issues whatsoever until a couple of years ago...
He is now on Clomid alone, so here's to hoping he starts producing sperm again. He's also started on 300mg CoQ10, 10,000 IU Vitamin D3, and 200mg zinc.
