Here are my results after 2 months of taking Natesto, and while on a low iron diet. I was also taking 18g of inositol a night.

No natesto on the morning of testing. Some days I would forget to take Natesto, or sometimes I’d double dose.

I have a separate lab for SHBG which turned out to be 56.

Estradiol may be lower due to using a tadalfil peptide.

Thyroid antibodies have been checked more than once in the past, and have been in range/checked by Dr Saya.

I have symptoms that are all over the place. I started waking up with morning wood random days just before Natesto. Might have a few rare days where I think I’m totally healed. Then back to experiencing symptoms.

Can have a terrible libido/ok orgasm or ok libido with sensitivity/ then terrible orgasm.

Brain fog some days and my brain doesn’t function well. Some days brain is super sharp.

Next step may be testing cytokines or taking HCG.

Do you think maybe you're free T is a little too low? What would happen if you increase your dose of Natesto. Or do higher levels cause you issues.

My baseline SHBG in December right before Natesto was 66. Now it’s 56, and 56 is about my average SHBG so not sure if Natesto is doing anything.

I could try more Natesto, but the bottle doesn’t last very long- may have to do 2 or 3 bottles a month. I’m almost out and I was too depressed to go to the follow up endocrinologist visit. Even if I get a refill, it’s 220 a bottle.

I think standard doing 11mg daily means 3 bottles will last 1 month. You prolly have room to increase the dose.

Not sure if I can afford 3 bottles a month, and hard to say if the doctor would even approve 3 bottles a month. Probably time to go HCG to finally rule out SHBG.

I’m going to be looking into pelvic floor congestion/ infection after reading an article.

Your not taking an AI? Weird how your E2 sen does not reflect your Testosterone. Obviously the high SHBG is lowering the Free T. I am not too familiar with Natesto, so I am unable to provide much insight. Could you be hypersensitive to the medication and causing higher than normal SHBG, maybe? For peptides well, you really don’t know what’s in them sometimes, over/under dosed, heavy metals, and other unknowns/variables (basing this statement from underground labs)

if anything I would just stick to a protocol for a while and try and be patient. It takes time for things like libido for instance, to balance out. Why are you taking Natesto compared to other conventional Testosterone replacement therapies? Do you have high BP, Hematocrit, RBC?

would be nice to see a full panel of everything before Natesto... LH/FSH, prolactin, cortisol, ACTH, DHEA-S, RT3, SHBG...etc.

Sensitive E2 before Natesto was 19. Peptides are notorious for lowering e2, plus it was tadalafil which lowers e2. I used it on a Saturday, felt great on Tuesday, then baselined and had the labs done Friday.

Natesto is a quick absorbing T that apparently won’t shut down natural T. I thought that it would boost DHT and e2, and buffer enough against any PDE5 inhibitor that could lower e2. No needles, no shutdown, and I can get it at the pharmacy.

I have the same panel + Vitamin D before Natesto. DHEA, cortisol and ACTH are a few years old, and results are normal range, but low end normal. RT3 is high.

gotcha so you have labs that show your HPTA intact from Natesto? LH & FSH are in good ranges i’m assuming.

Yes, baseline natural T is 700 with SHBG of 66, sensitive E2 at 19. I overdosed on a vitamin D supplement and blood tests reflect it. So vitamin D deficiency and high estrogen can be ruled out for high SHBG. LH and FSH always both around 6ish so I don’t bother testing it.

On regular Clomid and DHEA under Defy, my T was over 1000, SHBG of 30. I have not tried Enclomiphene yet.

that’s what I am saying lol... but i’m more curious and OP is too, to why SHBG is elevated. I’m sure the SHBG is skewing your estradiol levels along with Free T. Is this a common issue with Natesto or Cialis? Unless there is a supplement he is taking.

That seems to be the better route for you. Ask Dr. Saya for Enclomiphene, but this will only work if you are secondary of course. That is why I was curious about LH & FSH before the Natesto.

LH and FSH are normal before Natesto. It has been tested by various doctors over the years, and neither have been below 5

Did you have problems with high SHBG? Enclomiphene is going to probably bring my T up to 1000, like regular clomid did, but you said you had no sexual symptoms with T less than 100.

You'd have to still have good HPTA operation to get a total testosterone of 637 ng/dL on a day when you didn't take Natesto.

It’s decent, but I have the same free testosterone as someone with a T of sub 300 and normal shbg.

I’m really looking into this pelvic congestion caused by stomach/colon infections. Doing natural cleanses and stuff now. Congestion inflames the pelvic floor and surrounding tissue. I think those tissues are key to transmitting nerve signals. Sometimes I won’t feel the pain jolt if I hit my testicles. When I can feel that sharp pain, it’s goes up on the side way above the testicle.

Scheduling with Defy anyway so I can rule out shbg and low e2.

The proper way to take Natesto to achieve maximum effectiveness is that it needs to be applied 2- 3 times daily (22-33 mg/day).

Not once daily as it is pointless let alone you are not reaping the full benefits when taking it for trt.

This is not the case. Play around with the Tru-T calculator. With your numbers above your free testosterone is about 19 ng/dL, within the normal healthy range of 16-31. A guy with total testosterone of 300 ng/dL and SHBG of 30 nMol/L has free testosterone of only 9 ng/dL.

It’s the free T % that was similar. I’m not here to argue about that though.

I can be on one thread where people say T and free T don’t matter, then get clobbered over SHBG in this thread.
