Gentlemen, there is a cure for Low SHBG FINALLY. And, It is tangible unlike the "keto diet and fasting". There is a newly approved drug that could easily more than double our SHBG and for many of us would mean a new life and TRT working. The drug is called Rezdiffra (Resmetirom) manufactured by Madrigal Pharmaceuticals. There are two major problems: 1. This drug is costly 2. Pretty damn hard to get prescribed unless you have NASH. I don’t see a doubling of SHBG doing much of anything for someone with type 2 diabetes. You still have to fix the underlining metabolic issue. You have to fix what’s causing the low SHBG and treating the symptom and not the cause rarely works in the long run.

Is it possible that some people have low shbg and don’t have diabetes or some other disease causing it ? My shbg has been in the teens for a decade and I don’t have a clue what the underlying cause is other than some people are genetically predisposed to their shbg ranking when they take testosterone.

Absolutely. My SHBG has also run low for quite a few years now, and I’m about as far from being diabetic as u can get
 
A lot of people including myself have no signs of insulin resistance or T2D or Hypothyroidism but have low SHBG. It could be that the receptor in our livers for Thyroid doesn't get enough stimulation and we need something exactly like this. It is much safer than doing Superphysiological levels of T3 to fix our Low SHBG

A lot of people without metabolic issues have Low SHBG. And for many(Not All) it makes life on TRT hard .

I have low shbg and have been doing very well on the compounded cream. Injections weren’t the ticket for me. I was always blaming shbg and thinking I couldn’t handle higher levels. Until I switched to cream. Now my only issue is I’m kind of a hypo responder to empowers hrt base and they discontinued the hydrogel(Also still a cream) currently trying to get my dr to send my script to medquest for atrevis base.
 
Thanks for posting about this new medication. I plan to ask my endocrinologist about it. My SHBG level has been between 0-2 since going on TRT. Even before TRT it was between 10-14. TRT has helped me immensely. However, I've also always felt like I've been limited in the benefits I get from TRT, and have thought that my very low SHBG is one of if not the reason for this.
I've also asked my doctor about other medications, such as upstream and downstream chemicals, that being on TRT is hindering its effectiveness such as progesterone, DHEA, HCG, etc...but doctor has pretty much ignored or disregarded my questions about this.
Anyone have thoughts on other ancillaries such as DHEA, progesterone, HCG, etc that I should consider adding to my TRT to get better symptom relief and benefits from HRT? Energy and libido have improved and anxiety has reduced with TRT, yet I still do suffer from them, especially low energy levels and motivation. Thanks in advance for any helpful responses! I hope this doesn't hijack from OP, as not my intention and his post is really interesting and important to the discussion.

Me too, I can handle higher levels on shorter acting testosterone formulations, even 2000 peak on Jatenzo and 1052 midpoint. The higher that shot frequency on injections, the less Test I’m able to handle and the higher the side effects and the greater the drop in ferritin levels.

I had zero success with consistent libido and erections on injections beyond the first week. This isn’t a problem on Jatenzo.

My SHBG is 18-24 on Jatenzo and injections.

That’s great man. Everyone thinks guys like us are crazy (they right to a degree) but I’m glad your finally finding success.
